Description Family medicine PA/FNP caring for a diverse population of tribal members, veterans, and other Kodiak Island community members, dedicated to full-patient care across the age spectrum. Includes clinic, periodic home visits, and remote village travel. Will develop a patient continuity panel and care for this panel thoughtfully with colleagues' support. Care is delivered in a patient centered medical home and a team-based model of care. Provider will be well supported by an experienced physician staff, with at least one physician always readily available for clinic consults. Essential Duties and Responsibilities include the following. Other duties may be assigned. This provider will join an engaged and cohesive medical staff supported by a strong and thoughtful leadership team in the delivery of patient-centered care by evidence-based standards. The administrative support for this position is focused on provider health and support in the context of continuity family medicine delivered in a high-quality, patient-lead manner. We are dedicated to elevating the quality of life of our population and community members. Medical Services: Patient-centered family medicine provider caring for the Kodiak community, with special attention to the Tribal and VA populations. Provider will develop a continuity panel of patients across the age spectrum from newborn to geriatric. Provider will consult a physician when any unfamiliar, uncommon, or unstable conditions arise. Outpatient services are generally delivered in clinic and/or by telemedicine. Provider will periodically attend clinic in remote villages accessible by small plane, home visits, and other locations per request. Clinic visits will be acute and chronic care visits, with attention to caring for care team patients' needs. Clinic days will generally be scheduled 16 encounters / clinical day (4 clinic days weekly), although provider will be flexible to accommodate patient and team needs. Provider will answer nurse team clinical questions and review results during their clinical days. Focus on team-based, patient-centered care. Will work closely with RNs, MAs, front desk staff, and administrative support staff in addition to provider colleagues. Will also collaborate with other patient support services across the organization. Correctly and completely documents entries and maintains patient charts in a timely manner as outlined in the KANA Medical Clinic policies and procedures. Colleague support: Interacts with all colleagues and learners in supportive, productive manner. Providers lead our clinical teams through thoughtful communication with clinical and administrative colleagues and modelling of excellent customer service to patients and their families. Support of clinical colleagues in the delivery of patient care includes physicians, PAs, NPs, CHA/Ps, RNs, MAs, and other support staff in person, by phone, and through secure texting and EMR communications. Clinical colleagues often talk over cases together in a collaborative environment. Will support care team patients and colleagues as needed, including periodic inbox coverage when colleagues are unavailable. Will encourage and support colleague work-life balance while working as a team to meet patient and clinical care needs. Communicates effectively with patients, colleagues, and other KANA staff. Addresses patient and staff needs courteously and promptly. Education: Will participate in education of medical learners including medical students, PA or NP students, nursing students, and others. Will be an engaged and enthusiastic educator for our students along with the other providers. Will serve as a champion for family medicine when working with learners, and will intentionally demonstrate respectful and dynamic provider-patient relationships. Administrative: Proficient and efficient use of clinic EMR (Cerner). Timely electronic communication, inbox management, chart completion, results review & communication to patients. Remains current in the field of medicine and practices up to date medicine as established by national standards, with practical application based on patient preferences and clinical environment. Maintains licensure and other credentials as required. Will be a steward of healthcare resources by following protocols for patient specialty consultation and referrals within and outside the Alaska Tribal Health System. Works closely with the Purchased and Referred Care program and care team referral navigators as needed to ensure patient has access to most appropriate care. Accurately and promptly provides coding, billing or other financial information to KANA billing team and follows up insurance requests as required. Will be attentive to continuous quality improvement. Actively participate in and embrace KANA's continuous quality improvement projects and activities. Continually strive to improve patient experience and quality of care with attention to patient access, continuity, team-based care, patient-lead care, and community engagement. Participate in peer review with other providers. Monitors and shares necessary information to facilitate risk prevention activities with administrative team. Attends all OSHA, HIPAA, and other regulatory compliance training as directed or required. Represents KANA in a positive and respectful manner in all circumstances including with staff, community members, colleagues, and professional organizations. Coordinates and participates in community outreach programs, serves on KANA committees, fully engages in the health of Kodiak's community. Will often manage multiple tasks simultaneously, including telephone, computer operations and in-person visits while maintaining thoughtful communication with team members and patients. Supervisory Responsibilities This job has no formal supervisory responsibilities, but the provider will provide quality education of medical learners, and will be engaged in clinical quality management. The provider is a team leader, and will be expected to engage and work with their team as such. Requirements Current certification by the National Commission on Certification of Physician Assistants or Current certification by the American Academy of Nurse Practitioners or American Nurse Credentialing Center, (NP). Post training clinical experience in specialty preferred. Current Alaska license. Basic Life Support (BLS) certification is required. Physician Assistants must have collaborating agreement with current member of KANA medical staff. Must success fully pass the Indian Child Protection and Family Violence Prevention (ICPA) background check.
