According to the latest report from Rock Health, digital health funding in Q1 2024 totaled $2.7B across 133 deals, with an average deal size of $20.6M. It doesn’t break out device-related investments, but most medtech founders I meet these days are being forced to chase tiny rounds made up of endless little checks, consuming valuable energy and slowing their progress. The benign neglect of this next generation of diagnostic and therapeutic devices is counterproductive and surprising. Given the crowded virtual care market, for example, I’d expect more such companies would be exploring medtech partnerships, investments or acquisitions that'd enable them to back integrate into delivering sharper, and more differentiated, care and treatment. This is exactly what Maverix Medical is doing from the other direction: Reshaping the entire spectrum of lung cancer care by integrating more advanced + effective approaches to screening and diagnosis, treatment and therapeutics, and even palliative care…And putting it all to work through an innovative provider network. One notable investor exception is Y Combinator partner Surbhi Sarna who’s been backing great teams like Selera Medical (YC W24) doing foundational work on ambitious medical devices. Without more support from investors like her, we’re going to increasingly find ourselves stuck with the incremental portfolio improvements from the incumbents, and that’s just not nearly enough.

A few Friday thoughts... After several days in Chicago surrounded by 200+ LPs at the ILPA Members' Conference covering everything from AI to NAV facilities, I'm headed into the weekend energized by what's to come. That may come at a surprise after the week we've had as an industry. The decision from the U.S. Fifth Circuit on the SEC's Private Fund Advisers rules was handed down just moments before we kicked off ILPA’s AGM on Wednesday. All along, ILPA has viewed the PFA as addressing areas that pose risks to both LPs and the industry around transparency, governance and alignment of interest. It was disappointing—albeit not a shock—to see the PFA vacated. But ILPA is choosing to turn our energy to the compelling opportunity before us as an industry. Together, we can chart the path for how we'll work better together, on our own terms. Particularly around transparency. Just days prior to the ruling, we had debuted the next evolution of ILPA reporting templates on fees and expenses and performance for industry feedback. The templates were developed by a broad industry coalition over a three-month period, partly to comply with the PFA quarterly statements rule implementation deadline, but largely because this work needed to be done. We've temporarily paused the comment period. Now we have the benefit of more time to ensure that these minimum reporting standards have cross-industry buy-in to ensure wide support and adoption. The author advises founders to be cautious, focus on extending their runway, and prepare for a difficult period ahead. This will hit home in SE Asia in about 3-4 months. Get laser-focused, raise more than you need, control the spend and be realistic about your projections. Raise appropriately and be honest what kind of company you will be in 8-9 years. A trend that's happening is that a growing number of VC backed startups are creating MSOs (aka the PE model) to roll up independent practices in order to manage their operations and billing. Here's a thought. If IPAs (independent practice associations) operate like DSOs (dental support organizations) for dental practices, why can't they coordinate funding partners (e.g., credit unions) in order to purchase equity shares in the practices within the IPA? Doing this could keep the practice still primarily owned by the physician/s while providing cash (liquidity) where they don't have to sell to these MSO models or PE. I can see even the IPA starting its own fund to support this for the practices that are part of the IPO/A. So let's say there are 20 independent practices part of the IPA. The IPA starts a fund or gets access to funds to purchase equity/secondaries from the practices within the IPA. Shifting the responsibilities of the IPA from not just managing services but where the IPA becomes an equity owner vs just a management group. I don't see why the only options for independent practices is to either sell their practices to private equity or sell to VC backed MSOs. It actually makes more sense for IPAs (which is a PROVEN structure) to manage this. IPAs should evolve to provide funding for independent practices. Going this route helps practices truly stay independent while also providing value to the IPA itself. Like the DSO model, I can see all physicians of all the independent practices sharing a percentage of ownership for all of the practices within the IPA. What are your thoughts?
