Walmart will close all of its health care clinics Walmart, the largest retailer in the United States, will close all 51 of its health care centers in six states and end virtual health care services, the company said Tuesday. Walmart had made a big push into health care in recent years, opening clinics next to its superstores that offered primary and urgent care, labs, X-rays, behavioral health and dental work — Arkansas, Florida, Georgia, Illinois, Missouri and Texas. Walmart believed it could use its massive financial scale and store base to offer convenient, low-cost services to patients in rural and underserved areas that lacked primary care options. But the announcement is an abrupt reversal in Walmart’s strategy and may leave a gap in health care access, particularly for lower-income patients without insurance who relied on the centers. Walmart announced today that it is closing its 51 health centers citing a tough reimbursement environment and operational challenges. This, while CVS Health (with its acquisition of Oak Street Health) and Amazon (with its acquisition of One Medical) spear ahead. The original thesis based on offering access through its vast retail footprint was compelling but staffing these centers with medical personnel, all while operating efficiently to keep costs low within the current reimbursement model is a huge challenge, especially given the staffing shortages. https://lnkd.in/ezpJ4mmC With retail primary healthcare being cited as the promising model, there are so many considerations: 1. Acquiring a network of primary care practices is a quick way to gain access to physician networks 2. Simply offering access through a large retail footprint is not enough 3. Trust based relationships and patient education are necessary to serve a population that typically uses ER instead of primary care for medical needs 4 . Moving from fee for service to value based care model is necessary 5. Payors have an incentive and an advantage in growing the value-based primary care footprint 6. Retail health as we know it is dying. Walgreens just wrote off $6 billion in VillageMD goodwill. This week, Walmart exited healthcare completely, shutting down 51 centers. Simply put, VillageMD clinics and Walmart Health tried to bring a consumer experience to one of the lowest margin segments in healthcare, thinking there was business opportunity to be had by entering primary care. Why did their grand ambitions fall apart? Operating in standalone primary care is tough. The economics are truly unsustainable, unless you're 1️⃣ incredibly lean or 2️⃣ attached to a larger enterprise with material downstream value capture. In healthcare, primary care is a loss-leader, and that dynamic needs to change. Low reimbursement is exactly why operating an independent primary care practices is tougher than ever (a big reason why firms like Aledade exist). It's why we're seeing burnt-out physicians buck traditional pathways in favor of concierge medicine or direct primary care. And it's also why the urgent care → specialty services pipeline exists within our health systems. Coming back to Walmart, here are some working theories on what brought down the retail giant: 🏥 1: Building health clinics required significant investment and staffing. Organizationally, sounds like Walmart didn't want to devote more resources to a cash-burning operation outside of their core competency. 🏥 2: Walmart was either unable to justify the cost of standing up new clinics or couldn't quantify the downstream financial impact to pharmacy, vision, or retail sales. 🏥 3: Patients probably don’t want to get primary care at Walmart for anything other than a shot, preferring continuity of service. Finally, today's Medicare Advantage challenges likely worsened Walmart's book of business from a supplemental benefits standpoint (as some insurers are cutting benefits in a tough operating environment). It's interesting, because if the Fortune 1 company wanted to go all-in on healthcare, it could. But a lack of a cohesive strategy hamstrung its efforts to provide 'everyday low prices' in healthcare. I’d expect to see similar struggles for other retailers who, out of fear of missing out, joined the retail health bandwagon over the past several years but will likewise fail to capture downstream value. In short, this is another way to say: PCPs are saints, and we need to protect them at all costs along with finding reimbursement models that work to sustain their practices, rather than fragmenting healthcare further through poorly strategized retail health initiatives looking for a quick buck. Walmart is exiting healthcare. This doesn’t surprise me. Walmart was never designed for healthcare, it was designed for retail. Everyone wants a piece of the $4T pie, but it’s a pie full of indigestible ingredients that would make the biggest companies choke. Especially companies that were never built for healthcare. - fee-for-service doesn’t work, especially with very low fees - relying on legacy payors doesn’t work - using insurance for primary care doesn’t work - not paying providers enough doesn’t work - having a clinic next to gardening supplies doesn’t work Throwing millions at healthcare doesn’t automatically work with a large base that comes to you for something else. I believe a properly built and well-executed healthcare startup that “thinks differently”, will eventually move the needle on improving the American healthcare experience. It’s coming, and will surprise everyone.

"Honey, go to store for some milk, eggs, and bread, and while you're there get your cough checked out." Retail healthcare? Really? Major players like Walgreens (who acquired Village MD) are losing billions (that's billions with a B) by betting that Americans will want a medical office visit to be like picking up a 6 pack of soda. As a physician, I find this so insulting and disgraceful that I can't even understand it. I have to trust that the American people are not so easily fooled by this corporate charlatanism. Good quality medical care starts with a doctor-patient relationship. Period. There is no such thing as retail healthcare; it's just another example of corporate greed. I hope their enterprises fail and if Senator Warren gets her way, maybe their CEOs will do some time at Club Fed.

Recently there were some content circulating around the device "Pigg-O-Stat" on social media, and the comment section exploded, as usual. As a parent, it's almost unfathomable that I would need to put my feverish crying baby in this contraption for any reason. At the same time, I understand the importance of keeping the child still during the process so we can eliminate unnecessary radiation exposure and provide the sick child with the most accurate diagnosis and treatment. In the typical Piper+Enza fashion - we want to find the common ground and increase understanding from both POVs.
