Why is disaggregating health data important for everyone, not just a few? 🧐 Overly broad classifications often create gaps in how health data is collected and analyzed. For example, the commonly used "Asian American" demographic combines Chinese, Filipino, Vietnamese, Korean, Japanese, and South Asian communities. This aggregation often obscures significant health disparities between these socially and culturally distinct groups. Tuyen Pham Anirudh Ruhil and I recently published an article estimating the opportunity costs of COVID-19 deaths in the US. The opportunity cost of COVID-19 deaths is defined as the combination of direct medical costs and the costs of lost potential lifetime earnings. The Commonwealth Fund released a new report with alarming statistics on health disparities in the U.S. Even in states considered to have high health care performance, such as Rhode ​Island, Massachusetts, and Connecticut, the report found stark disparities. It ​called out that white and Asian residents had the best health outcomes. The report calls for several policy initiatives with consideration including reducing barriers to health care coverage, access to Medicaid, and bolstering support for addressing social needs. At Kaiser Permanente, my colleagues and I are working to make high-quality, equitable health care a reality for all. Over the past 2 years, the Urban Institute has completed an impressive body of work focused on the question: How can we make substantial progress toward eliminating racial disparities in health and ensuring equitable care for all patients?  Our products include a 2023 symposium, papers and guides, and virtual dialogues. Current efforts include research on using demographic data to advance health equity, developing new research strategies to bolster equity initiatives in the wake of recent Supreme Court rulings, and preparing for the 2030 Census. As Tony Iton and Damon Francis have written, "The fractures in the US health system—and in the country’s social contract—can be fixed. I am excited to share with you my first published paper, focusing on a critical public health matter 📚 🚨 Safety Net Hospitals (SNHs) across the country are facing severe financial distress, leading to alarming closures that exacerbate health disparities in underserved communities. These closures are largely driven by the current reimbursement model maintained by The Centers for Medicare and Medicaid Services (CMS). SNHs play a vital role by providing specialized services and acute medical care to populations encountering structural and systemic healthcare barriers. Their survival is crucial to ensure equitable access to quality healthcare for all. 🔎 Our paper, featured in the Journal of Community Health Equity and housed in U.S. National Library of Medicine, explores a Total Cost of Care (TCOC) financial model as a potential solution to this critical issue exacerbating health disparities in the U.S. The model aims to address the needs of healthcare providers while supporting the complex medical and social circumstances of underserved communities. 🏥 A huge thank you to Shanya G., and James Gasperino, MD, PhD, MPH, for their incredible insights, agility, and dedication throughout this journey. "It Is Time to Consider More Price Regulation in Health Care." My favorite excerpt below: Research on hospital productivity in Medicare finds that hospitals often misallocate their resources, and the hospitals that spend less may have outcomes that are as good as the hospitals that spend more. In the commercial market, hospitals paid higher prices do have better mortality outcomes, but that is only true in less concentrated markets. In markets with little competition, hospital prices are high but quality is lower than in competitive markets. Today, our Boston Strategic Partners team (Hal Yapici, MD, MBA, MPH) at #ISPORAnnual presents the poster (MSR6) entitled “Regional Dynamics and Prevalence of COVID-19 Variants in the U.S. Health and Human Services Regions” ISPOR—The Professional Society for Health Economics and Outcomes Research. Our team utilized #MachineLearning to understand various regional and temporal factors on percentages of key #COVID19 variants. This study aims to uncover predictors of variant prevalence and contribute to a more data-driven approach to #PandemicManagement. Using data from Bureau of Transportation Statistics (BTS), US Environmental Protection Agency (EPA), the U.S. census, and the National #COVID Cohort Collaborative, we created a #predictivemodel using #RandomForestRegression. We sought to analyze how different factors impact #COVID-19 variants such as #Delta, #Omicron BA-5, #Alpha, and XBB. 1.5. The model demonstrated high predictive accuracy, with R² values of 0.99 for Delta and BA.5, 0.98 for Alpha, and 0.97 for XBB.1.5. significantly surpassing the 0.78 R² value for a mixed-variant baseline. The spread of Delta correlated strongly with #OzoneDensity, BA.5 with sun hours and #UVIndex, Alpha with temperature and #AirQuality, and XBB.1.5 with land area and income. These results suggest a complex interplay between environmental factors and variant spread. This study shows that machine learning is useful for identifying the multifaceted contributors to COVID-19 variants. The results may shed light on targeted public health interventions and policies, highlighting the vital role of data-driven models during a pandemic. Hot off the press: a new article from the Lancet: Digital Health published last week explores how a team at Brigham and Women's Hospital tested using Chat-GBT (a large language model, or LLM) to assess how LLM's might be used to respond to patient portal messages, and how this might impact subjective efficiency, clinical recommendations, and potential harms. TL;DR: using ChatGBT reduced physician workload, improved consistency across physician responses, and enhanced educational value of responses. However, there are ethical concerns with LLM's biasing physician clinical decision making, and that "physicians might use LLMs' assessments instead of LLM responses to facilitate the communication of their own assessments" So your MyChart message might be answered by a robot from now on, and that robot might be the one making clinical recommendations for you...
