ANCHORAGE, Alaska, #April #29 #2024 #Ideal Option, a #national #leader in outpatient medication-assisted treatment for addiction to opioids and other substances, has released its #2023 #Annual #Patient Outcomes Report for patients #living in Alaska. This report contains an aggregated #view of patients' geographic and demographic characteristics, their #stage and duration of #care adherence to medication and their substance use over time based on lab specimens collected during 2023 from nearly 1,500 patients. The report confirms the rapidly changing trends in street drug formulations; notably the increasing prevalence of synthetic opioids such as fentanyl and cutting agents such as xylazine. It also confirms that polysubstance use is now the norm, making treatment more complex. Data show that 69% of all Alaska patients were ingesting more than one substance at the time of enrollment, and 41% tested positive for three or more substances. In a recent blog post, Erkan Buyuk, M.D. breaks down surrogacy — from when it could make good sense for patients to pursue to what can be expected in the process: 🔵 A gestational surrogate or gestational carrier is a woman who carries a pregnancy for another person or couple who is (are) the genetic parent(s) and with whom she is not in intimate sexual contact. Historically, the term gestational surrogate was used for women who contributed the egg and also carried that pregnancy whereas gestational carriers only carried the pregnancy without donating their eggs. However, when the laws were changed and contract arrangements for gestational carriers became legal in New York state in February 2021, the term used in the law was “gestational surrogate”. This individual is defined as a woman who carries the pregnancy for another person or couple but doesn’t have a genetic connection to the fetus that she carries. 🔵 A gestational surrogate is needed in many clinical circumstances: women who do not have a uterus from birth or who had their uterus removed for various reasons and want to have a child; women who have severe chronic conditions that preclude them from getting pregnant, such as pulmonary hypertension, severe heart disease; women who have to take medications that can’t be used during pregnancy; and gay single men or couples who wish to have their own biologic child(ren). *Regenerative Medicine Pearl* After about 30 messages yesterday, mostly from the Phoenix Arizona area, inquiring about how to report potentially noncompliant USA regenerative medicine clinics or providers to the FDA or FTC....I wanted to put this out there for reference. If a patient, or really if anyone, feels there is a need to report egregious marketing practices (print, internet, social media, etc.) the link to report to the FTC is below.....it is online and takes about 5-10 minutes. Systematic Review Highlights Fertility-Sparing Treatment Efficacy in Large Cervical Cancers A recent review examines the reproductive outcomes for women with cervical cancer larger than 2 cm who underwent fertility-sparing treatment (FST). The review analyzed data from 17 papers, including 443 patients who had undergone FST, with 80 pregnancies resulting in 54 live births and 24 miscarriages. The study suggests that FST can be a viable option for childbearing women with larger cervical cancers, but further research is needed to understand the balance of treatment efficacy and potential risks.
