Elevating Chronic Care Management through Nurse Care Managers As healthcare continues to pivot towards value-based care models, the role of #NurseCareManagers (#NCMs) in Chronic Care Management (#CCM) has never been more pivotal. NCMs are at the forefront of identifying eligible patients, developing personalized #careplans, and ensuring seamless #carecoordination. But how does this translate into better Quality Payment Program (#QPP) scores and increased incentive payments? First, by identifying patients eligible for CCM, NCMs ensure that those with multiple #chronicconditions receive continuous, comprehensive care. They not only enroll patients but also provide them with the necessary education about the benefits of such programs, leading to higher patient engagement and satisfaction. Secondly, care coordination is the heart of CCM. NCMs work tirelessly to connect the dots between various healthcare providers, ensuring that the patient's care plan is executed flawlessly. This includes managing medication, scheduling follow-ups, and monitoring the patient's health status. The seamless execution of these plans leads to improved patient outcomes and reduced hospital readmissions, which are critical metrics in determining QPP scores. Lastly, meticulous documentation and correct coding of CCM services by NCMs ensure that providers are reimbursed accurately for the care provided. This not only supports the financial health of the practice but also reflects the quality of care in QPP scores. In essence, NCMs are invaluable in navigating the complexities of chronic care, enhancing patient health outcomes, and steering healthcare practices towards higher QPP scores and increased incentives. Three years ago, Centers for Medicare & Medicaid Services Innovation Center set a goal of having all traditional Medicare beneficiaries and the majority of Medicaid beneficiaries in accountable care by 2030. Read this promising progress report in Health Affairs from Liz F. and her colleagues.    ✅Integrated care empowers patients to be active in decision-making about their health    ✅When care is coordinated, primary care providers and specialists can share insights and treatment plans, which reduces fragmentation and improves health outcomes   ✅ CMMI will continue to update condition-specific models to target areas where patients experience gaps in high-value care. Prioritizing condition-specific models will allow specialists to take a more principal role in patient’s care and focus on their specific needs.    It's been a privilege to support patients in CMMI's Kidney Care Choices model with the Patient Activation Measure (PAM). Behavior Analytic Colleagues!  ABA Progress Report for Health Professionals. An important collaboration orchestrated by the NYS Association for Behavior Analysis with the Developmental/Behavioral Pediatric community (as appropriately represented by Dennis Kuo, M.D., M.H.S, Chief of Division of Developmental and Behavioral Pediatrics at The University of Rochester Medical Center/Golisano Children’s Hospital) has resulted in a wonderful and simple form for Behavior Analysts to report to health professionals on therapeutic goals and progress for those with whom we share the care. NYS requires Behavior Analysts to report to “ABA prescribers” on an annual basis; while this form was developed to help NYS LBAs meet this licensure requirement, the basic structure and design of the form can serve the needs of Behavior Analysts everywhere. As a pediatrician -- and NYSABA member -- who works with both autistics and people with intellectual and developmental disabilities, this form provides an optimal framework to share summaries of goals, progress, and ambitions for therapy. This form can also serve as an important vehicle for education about behavior analysis; it is less about the details and more about the trends and aspirations – and could also help build better bridges between the child health and behavior analytic communities.
