A discussion on the ‘right’ MBA for an MD… “While I was at KFSH&RC (the King’s hospital) in Saudi Arabia, a new Saudi anesthesiologist who had trained in Canada asked me how I knew so much. I told him it was because I had an MBA and learned a lot of process concepts. He might have been talking about the pediatric sedation system that I set up at KFSH as well… No one had ever seen anything like it before, whether the technique for that type of treatment, or the data recording system. I did it for the head of radiation oncology who was a Kiwi friend of mine after we had talked about how he was sedating the children for the radiation therapy. It took me 20 minutes of talking with the techs and looking at their ‘constraints’: access to airway of patient; non-moving; length of process; positioning and their equipment. We agreed upon some head constraint modifications and sequencing. A few days later I returned to see the modifications and review the process. The next week I ran through a couple of days of doing 4 patients each day, then accompanied my anesthesia colleagues there to show them the process in real time. It made the process so much faster that were enought sessions that my radiation colleague wrote a paper on the result of the radiation therapy for the patients. There was a tremendous effect on the immediate family since the children often had needed to be admitted overnight from the prior sedation technique. The mothers at that time usually had 5 or 6 young children, and had to take care of them, too. It may have saved a few marriages, and had a significant psychological effect on the other children. There was also a necessary change in the normal anesthesia chart for each patient. It normally would have taken at least 20-30 minutes to record the preop and standard anesthesia record. I realized doing the first case that with all these patients having 5 or 6 repeat sessions that the preop info would always be the same, and the most important part for each anesthesiologist on the next episode was to know how much repeat doses of propofol were given, the spacing, and the reaction by the patient. The system that I set up had all that info available immediately, so we could keep eyes on the child and input the new data in 3 seconds below the prior data. Also, there was a shortage of medical record personnel so that many anesthesia and event record could not be retrieved easily. My system permitted all the record to be kept in a room next to the procedure room. It was simple to pick the right patient’s folder and use it. So…my process also saved the medical records personnel time which permitted them to help other doctors who needed access to their patient records for what every reason. Lots of connections and effects that they never realized. It just made everyone a lot happier.”

Many physicians and nurses are happy about the Federal Trade Commission’s new rule banning the use of noncompete agreements in employment contracts. ULTRA RAPID CORONARY ARTERY DISEASE PROGRESSION Presenting a case of 45 year male who underwent coronary angiography for POSITIVE TMT showed a non-critical coronary tree, however discharged on plaque stabilizing drugs he presented to ER within 2 months for an acute AWMI and to surprise had an 99% thrombotic stenosis of LAD. This brings us to the question of vulnerable plaque as "plaque rupture" was the etiology in my case. The recent PREVENT trial showed benefit of preventive PCI in ACS patients with >50% stenosis, FFR negative with vulnerable plaque (including IVUS MLA <4.0mm2; plaque burden >70% ; NIRS LCBI>315; TCFA). But the inclusion criteria was not meet in the mentioned patient as the stenosis was not >50% angiographically. Heigly vascular mets like RCC and thyroid ca. If no urgent intervention is must, better to go for embolization preoperative first.

Ann Card Anaesth. 2024 Apr 1;27(2):136-143. doi: 10.4103/aca.aca_204_23 Background: Prolonged preoperative fasting may worsen postoperative outcomes. Cardiac surgery has higher perioperative risk, and longer fasting periods may be not well-tolerated. We analysed the postoperative metabolic and hemodynamic variables in patients undergoing elective coronary artery bypass grafting (CABG) according to their morning or afternoon schedule. Methods: Single-centre retrospective study at University teaching hospital (1-year data collection from electronic medical records). Using a mixed-effects linear regression model adjusted for several covariates, we compared metabolic (lactatemia, pH, and base deficit [BD]) and haemodynamic values (patients on vasoactive support, and vasoactive inotropic score [VIS]) at 7 prespecified time-points (admission to intensive care, and 1st, 3rd, 6th, 12th, 18th, and 24th postoperative hours). Results: 339 patients (n = 176 morning, n = 163 afternoon) were included. Arterial lactatemia and BD were similar (overall P = 0.11 and P = 0.84, respectively), while pH was significantly lower in the morning group (overall P < 0.05; mean difference -0.01). Postoperative urine output, fluid balance, mean arterial pressure, and central venous pressure were similar (P = 0.59, P = 0.96, P = 0.58 and P = 0.53, respectively). A subgroup analysis of patients with diabetes (n = 54 morning, n = 45 afternoon) confirmed the same findings. The VIS values and the proportion of patients on vasoactive support was higher in the morning cases at the 18th (P = 0.002 and p=0.04, respectively) and 24th postoperative hours (P = 0.003 and P = 0.04, respectively). Mean intensive care length of stay was 1.94 ± 1.36 days versus 2.48 ± 2.72 days for the afternoon and morning cases, respectively (P = 0.02). Conclusions: Patients undergoing elective CABG showed similar or better metabolic and hemodynamic profiles when scheduled for afternoon surgery.

Log-normal distributions frequently are suitable anesthesia endpoints: times until patients are ready for post-anesthesia care unit discharge, anesthesia times, surgical times, extubation times, intraoperative notification response times, and times for placements of regional blocks. However, after completing the study, Weibull distribution fits are sometimes comparable or better. What's the consequence? P-values get larger and confidence intervals wider (i.e., can trust P <.05 as significant but ahead plan slightly larger sample sizes). https://lnkd.in/gF8sMcr9
