About us
Moments Hospice is Medicare Certified Hospice Agency servicing most of Minnesota, Wisconsin, Chicagoland, Miami, Sioux Falls and Des Moines metro areas. We are focused on creating individual plans of care, because every patient and family is unique. Moments Hospice is committed to honoring patient’s and family’s wishes during the end of life process. We take great pride in creating long lasting partnerships with our Hospitals, Facilities and Community partners by recognizing that we are guests in their facilities and that collaborating on a joint plan of care always results in the best outcome for the patient. It is our mission to help increase the communities awareness of the Hospice Benefit as we believe that everyone deserves to be comfortable with an optimal quality of life during the end of life process. Rather than a place, hospice is a philosophy - a program of care and support wherever patients need us. Our services extend to any location - a private residence, hospital, assisted living facility, or nursing home. Moments Hospice has defined as its mission to affirm life during its final stages by providing compassionate care to patients and their families; by providing access to hospice care for underserved or difficult to serve populations; by wise and efficient use of available resources, and by educating the community in order to provide them with knowledge regarding end-of-life and hospice care around quality of life.

During National CNA Week, we extend our heartfelt appreciation to outstanding individuals like Kari Steele, whose dedication and compassion brighten the lives of our patients at Moments Hospice every single day. “My inspiration that led me to Moments Hospice is my parents. I lost my father in 2001 and my mother in 2023. They are my reason and why I strive to provide quality care to my patients and support to their families during their hospice journey. As I reflect back on some of my most meaningful experiences, it's hard to identify just one because they are all memorable and meaningful. Especially touching are the moments when I get to know a patient through stories shared by themselves or their family and friends before their journey begins. Some of the encounters I've been a part of include dressing a final moments patient in a Christmas nightgown so she could attend her great granddaughter's school holiday concert, being thanked by a family for making their mother beautiful before she reunited with their father in Heaven, and helping a bedridden patient into a reclining wheelchair so she could enjoy the warm sunny breeze outside with her sister on their acreage. These are just a few of many occasions that remind me why I LOVE MY JOB. I bring empathy and compassion to my work, caring for patients the way I would want to be cared for.
