Delivery System Reform (DSRIP)

Communities Together for Health Equity

The Delivery System Reform Incentive Payment (DSRIP) Program is a $7.4 billion, five-year Medicaid funding program based on an agreement between the State of New York and the Federal Center for Medicare and Medicaid Services (CMS). The DSRIP program began in April 2015, with an overall goal to reduce avoidable hospital use by 25% (including unnecessary visits to the emergency department) by offering funding to transform the health system for Medicaid members and the uninsured.  DSRIP projects are designed to improve integration and coordination with the health care delivery system, to build up community-based health care capacity, and to better address the social determinants of health (SDH).  SDH are the conditions in which people are born, grow, live, work and age, and the wider set of forces and systems shaping the conditions of daily life.  Organizations that support economic opportunity, empower minority and immigrant populations, support individuals with disabilities, and promote safe housing, education, workforce development, environmental justice, food security, etc. are addressing the social determinants of health.

CTHE Model

A model was created to deeply engage community based organizations (CBOs), and to consider their integral role in reducing avoidable hospital use. The Communities Together for Health Equity (CTHE), a consortium of NYC community organizations and stakeholders, was created, and The Arthur Ashe Institute for Urban Health (AAIUH) was selected as the lead organization. cthe.us to learn more.

Goals & Objectives

Our goals and objectives include continued formation of a sustainable and robust CBO Consortium engagement. The long-term vision of this work is to concretize a sustainable infrastructure and process that can demonstrate CBO value, experience, and impact, in order to inform DSRIP and ensure CBO engagement in a health care delivery system that is directly responsive to community needs.
