New and Returning Students

New and returning students for the Fall 2024 semester must complete their wellness requirements (including uploading immunizations and enrolling in or waiving out of the Penn Student Insurance Plan) starting July 1.

Student Health and Counseling: Modified Summer Hours

Student Health and Counseling will have modified hours for the summer (through mid-August). Students can always reach a provider 24/7 via phone by calling 215-746-WELL (9355).

Access your Student Health and Counseling records (from after January 2024) and upload and manage your immunization records. Please note: you will be directed to your PennChart through myPennMedicine.

Health Records (Prior to 2024)

Please fill out this request form in its entirety, including the special records section and chose one way to have them sent to you. If a form is not completely filled out, we will need to contact you which may delay the processing of your request. Please allow two weeks for records to be sent.
