If you are requesting permission to reprint from two or more of our books, please complete one form per title. Please note that we cannot grant permission to reproduce material that we have reprinted in our books by permission of another rights holder. It is your responsibility to determine the correct copyright holder using the credit lines and acknowledgments in the book and to apply directly to them for permission.
