The prevalence of aortic stenosis is increasing due to aging of the general population. Transcatheter aortic valve implantation (TAVI) is an alternative for surgical aortic valve replacement (AVR) for high-risk patients. Cardiac scoring systems have been developed to predict the risk of adverse outcomes following surgery, but currently they do not include frailty parameters, and frail patients have an increased risk of adverse health outcomes. Preoperative frailty correlates with adverse outcomes, mortality and functional decline in patients undergoing TAVI; by identifying these patients it is possible to counteract symptoms of frailty by implementing tailored intervention by the geriatrician. The Comprehensive Geriatric Assessment (CGA) is a tool used to detect frailty. Based on the current literature, we highly recommend performing a CGA and a minimum of a Timed Up and Go test, a mini nutritional assessment and serum albumin measurements in patients aged > 70 years undergoing a TAVI or surgical AVR, in order to aim reducing the accompanying risks of the intervention.

Available clinical balance tests determine the patient’s ability to maintain standing balance under defined test conditions and aim to describe the current state of this ability. However, these tests do not reveal which of the underlying systems is deteriorated and to what extent, so that the relation between cause and effect often remains unclear. Especially detection of early-stage balance control deterioration is difficult, because the balance control system is redundant and elderly may use compensation strategies. This article describes a new method that is able to identify causal relationships in deteriorated balance control, called CLSIT (Closed Loop System Identification Technique). Identification of impaired balance with CLSIT is a base for development of tailored interventions and compensation strategies to reduce the often serious consequences of deteriorated balance control in the elderly.

Impaired standing balance is highly prevalent in elderly people, leading to increased risk of falling. These falls can result in fractures and mortality, and therefore have a major socio-economic impact. Maintaining balance requires a concerted action of the sensory, nervous and motor systems, whereby cause and effect mutually affect each other within a closed loop.
In clinical practice, the underlying mechanisms are hard to disentangle. A variety of balance measures are available including clinical and instrumented balance assessment. Their interrelation is yet unknown as well as validity with respect to experienced balance impairments and falls, which will be tested in 'A matter of balance'.

Thus, understanding the many factors that contribute to healthy ageing versus frailty, and validating interventions and influencing policy to promote healthy ageing is a cross-cutting research priority in Europe. Given the considerable impact of lifestyle factors on healthy ageing and disease, there is a surprising lack of innovative multi-disciplinary training and research examining the influence of physical activity and nutrition on age-related changes at gene to societal level. PANINI will address this gap by coordinating research laboratories across Europe to focus on cutting-edge ageing and health research through training 11 ESRs across scientific disciplines to create a holistic approach to the challenge of ageing in the 21st
Century. PANINI is a European Training Network with 8 world-leading beneficiaries working on Healthy Ageing and 10 non-academic partners carefully selected for quality and range of sectors. These vary in size including 1 large nutrition company, 5 SMEs, 2 charities, 1 healthcare partnership and 1 EU Joint Research Centre. PANINI’s vision is to implement multidisciplinary cross-sectoral research and training of a new cohort of researchers taking a radically innovative approach to
overcome the challenge of unhealthy ageing. The main goals of PANINI are to: integrate expertise from basic biomedical science and technology to applied clinical practice; standardise measurements across the network to create a toolkit and shared dataset; provide multidisciplinary training secondments to expose ESRs to healthcare and industry research settings and other laboratories; disseminate widely the shared and individual project findings; and use these to develop a policy document to promote healthy ageing in Europe.

Lifestyle, disease and biology put older people at risk of functional decline, leading to falls, cognitive impairment, frailty, and negative consequences for quality of life. PreventIT will develop and test an ICT based mHealth System (iPAS) for the consumer market that 1) enables early identification of risk of age-related functional decline, and 2) engenders behavioural change in younger older adults in order to adopt a healthy, active lifestyle. We will use an integrated system of a smartphone/watch as frontend technology and a protected cloud-based solution for handling of personal data as backend technology. We will develop online instruments for risk-screening, complexity metrics, motivation for behavioural change, and a method for personalise exercise by phenotype, based on currently available big data sets. The ICT based intervention we develop is unique, delivered on a smartphone/watch with exercise integrated in daily life and a behaviour change programme. We will evaluate the role of technology and behavioural change theories in risk prevention by performing a
multinational feasibility RCT to compare with the same intervention without use of technology and with practice as usual. Usability and feasibility will be in focus, and ageing people will be included in all phases of the project. At the end of the project all assessment and intervention components will be integrated into a personalised self-administered activity system, designed to empower ageing people to control their own health and function. A microsimulation health economics model will
be developed to predict effects of our risk prevention intervention on carers and the health care system. PreventIT will be performed by a strong interdisciplinary team with previous impact in the field of ageing and ICT, and commercial partners who are strong on technology development and exploitation are included to increase the chances for iPAS to become a first choice mHealth system for the ageing generation.Other creators -

Sarcopenia, low muscle mass, is one of the undesirable factors leading to physical disability, poor quality of life, loss of independence, and death in older persons. We aim to understand the interrelations between muscle related parameters (muscle mass, muscle strength, physical performance), energy expenditure (actual physical activity and basal metabolic rate), and nutritional status (energy and protein intake vs. needs) in geriatric outpatients and healthy aged matched controls.

We willen een goed werkend brein, maar verlangen niet terug naar de onzekerheid van de pubertijd. Prettig oud worden is dus niet zomaar hetzelfde als jong blijven.
