The National Health Policy “Dr. Juan Guillermo Ortiz Güier”. “…inspired by the Hospital Without Walls (“Hospital Sin Paredes” in Spanish). The care model, which was developed in 1955 by Dr. Ortiz Güier, was consolidated in the 1970s in the Western Region of #CostaRica, and the World Health Organization (#WHO) in its Alma-Ata Declaration of 1978, called it primary health care. According to the Costa Rican Health Account System Reports, the average/year distribution of Caja Costarricense de Seguro Social (CCSS) expenditure for the Health Care Areas or Primary Care Level increased by 14%, from 25.08% for the year group 2011-2016, to 39% for the period 2017-2019, compared to the percentage assigned to Hospitals and Specialized Services (Second and Third Care Levels together), which in the same period fell from 64.08% to 49.75%, respectively.”

Let’s talk about Cross-Mentorship. As part of a conscious strategy, I mentor people at my company - Two Chairs - who do not work for me. Likewise, the Chief Product Officer David Covell, Chief Operations Officer Justin Wu, and Chief Commercial Officer Alfonso Castillo mentor members of my team. We do this every month. Why do we do it and why should you give it a shot? As Peter Drucker once said, “culture eats strategy for breakfast”. This has always resonated with me. When you are trying to merge two very different cultures, like clinical and tech, you need to embrace both so it doesn’t feel like oil and water. What it means in practice is that each team recognizes that it has strengths and weaknesses that other teams can learn from. Our product team can help the clinical team learn project management, root cause analysis, data informed performance management, and how to bring data and software to the solution. On the other hand, the clinical team can help the product team understand the clinicians, the clients, the work of therapy. They can also leverage our clinical and clinical leadership approaches, like compassionate leadership or Motivational Interviewing leadership with their own teams. We learn from each other - and we’re doing it every month. Why not give it a try within your own organization. I can promise you, the impact will be immense.

Walmart is exiting healthcare. This doesn’t surprise me. Walmart was never designed for healthcare, it was designed for retail. Everyone wants a piece of the $4T pie, but it’s a pie full of indigestible ingredients that would make the biggest companies choke. Especially companies that were never built for healthcare. - fee-for-service doesn’t work, especially with very low fees - relying on legacy payors doesn’t work - using insurance for primary care doesn’t work - not paying providers enough doesn’t work - having a clinic next to gardening supplies doesn’t work Throwing millions at healthcare doesn’t automatically work with a large base that comes to you for something else. I believe a properly built and well-executed healthcare startup that “thinks differently”, will eventually move the needle on improving the American healthcare experience. It’s coming, and will surprise everyone.

So far, it's the cottage industry of companies that are implementing changes. Further, these impacts are disproportionately felt among Black Americans. Want the inside story on what AI tools are being piloted at ScionHealth? This week I chatted with VP of Strategic Operations & Initiatives, Sarah Hughes, MSHA, MBA about how they’re using Eon, LUMINARE, and Operait Health to build out their AI toolkit. — 🟩 Q: What are your biggest tech-related priorities for 2024? 1) Workforce support—How can we deploy technology solutions that make our staff’s lives and daily work easier and more efficient? For example, we are currently deploying a technology platform, M7 Health, which has built a dynamic nursing workforce management system to help us understand and optimize each nurse’s work-life preferences with regard to scheduling and career goals. 2) Patient activation and engagement—How can we better engage and activate patients on both the front end (i.e., scheduling appointments and interacting with our organization) and the back end (i.e., continuing to engage with us to manage their health)? 🟩 Q: Where are you currently exploring AI-driven solutions? We’re running pilots to address a few different administrative and operational efficiency use cases: Incidental findings on radiology reads—We’re working with Eon to automatically scan radiology reads to identify and manage patients with incidental findings, and proactively reach out to them to schedule follow-up care. Sepsis early detection—We’re currently testing LUMINARE's platform, which implements the hospital’s sepsis protocols by automating intervention, streamlining communication, and simplifying reporting and analytics. Operating room scheduling—We’re also looking at a vendor called Operait Health which has built an AI-first solution to optimize surgical demand and right-size OR staffing by forecasting gaps in the OR schedule and placing cases unrestrained by blocks. 🟩 Q: Which areas are you most excited about the potential for new solutions? We’re excited about the potential for virtual remote care in the home on the long-term acute care (LTAC) side. We’ve already seen success with these models in our community hospitals, and we believe there’s potential to similarly deliver a better patient experience at lower cost by bringing care into the home for a subset of our LTAC patients. 🟩 Q: Any general advice for healthcare IT leaders out there? It can be easy to underestimate the legacy IT “tax” of bringing in any new vendor, given that any new solution must work with our existing tech stack. Vendors need to factor this in when discussing implementation timelines, and health systems need to ensure they include all the right internal stakeholders, including operational leaders, early in the conversation to appropriately plan for integrating and deploying solutions into day-to-day operations. — p.s. I loved this conversation because of all the practical wisdom Sarah shared. We're on the lookout for similarly insightful health IT leaders to feature. Who should talk to?

As we wrap up National Minority Health Month, it’s a good time to recognize the importance of addressing health disparities and ensuring equitable access to quality care for all communities. As a Latina woman leader in healthcare, I'm particularly passionate about this cause. Here at CareNu, health equity is a top priority. We’ve developed a Social Determinants of Health survey framework that’s continuously improved through artificial intelligence technology. Our survey found that access and independence were the two greatest barriers to care, with 80% of respondents reporting difficulties running errands like doctor visits, while 63% have trouble completing medical forms. CareNu confronts these challenges in a myriad of ways. We embrace the cultural preferences of different ethnic groups, targeting populations to address language and other barriers. We become advocates to help populations integrate into their communities and receive the healthcare they deserve and need. The results speak for themselves. We achieved a 14.7% reduction in social risk based on SDOH interventions in Black and African-American populations in just one year. We’ve also achieved a 6% decrease in the impact of psychosocial health risk factors, a 14.3% decrease in risk for patients in assisted living facilities and skilled nursing facilities and a 34% decrease in risk for patients in rural communities over a one-year period of time. With these breakthroughs in mind, I'm reminded of the strides we've made and the work that lies ahead.
