The Health Disparities and Community Grant Program (HDCGP) is funded by the Colorado state tobacco tax revenue as mandated by Amendment 35 (A35) and by funds appropriated by the general assembly as mandated by Colorado SB21-181. The program typically conducts its funding cycle according to the state fiscal year (July 1 – June 30). All funding is contingent upon Colorado State Board of Health approval, spending authority of the program and appropriations by the state legislature each year. The Health Equity Commission assists in the review of the grant program.

Amendment 35 funding opportunity overview
The Colorado Department of Public Health and Environment (CDPHE), Health Disparities and Community Grant Program (HDCGP) is pleased to announce the Amendment 35 (A35) grant opportunity due Monday, August 19, 2024 by 5:00 p.m. (MT).
The HDCGP-A35 grant supports community projects aimed at preventing, detecting and treating cancer, cardiovascular, and chronic pulmonary diseases among underrepresented populations in Colorado. The grants focus on addressing systemic health disparities and improving health equity by tackling physical, and social environmental challenges, including barriers associated with accessing timely and preventive health care.

Applications are due by 5 p.m. MT, Monday, August 19, 2024.
At precisely 5:01 p.m. on Monday, August 19, 2024, the online survey will lock and no further PAR-RFA submissions will be permitted.

Who should apply
Eligible entities must be Colorado-based organizations listed below that are committed to serving diverse, underrepresented, and vulnerable communities in Colorado.
- Grassroots organizations.
- Non-profit organizations, including healthcare non-profits.
- Public educational institutions.
- State or local governmental agencies.
- Federally recognized Native American Tribes.
- Organizations serving Native American communities, including those on reservations or Tribal lands, with a signed letter of support from the applicable Tribe’s leadership.
Note: For-profit organizations are not eligible.
Project priorities
All proposed projects for funding must be aimed at preventing, detecting and treating cancer, cardiovascular, and chronic pulmonary diseases among underrepresented populations in Colorado.
Funding under this RFA emphasizes collaboration and inclusivity, requiring partnerships across various sectors to effectively address the needs of underrepresented and vulnerable communities.
Grant funding phases
This grant funding process will take place in two phases:
Phase 1: Pre-Application Requirement for Request for Applications (PAR-RFA)
This is a competitive phase open to all interested entities deemed eligible as indicated in the PAR-RFA. All PAR-RFA submissions will be reviewed and scored to arrive at select finalists that score high enough into a competitive range as defined by CDPHE.
