In an effort to protect your family and other attendees, we ask that all attendees (12 years old and older) have proof of full vaccination or a negative COVID test within 3 days of the event. Image of vaccination information or test results on mobile devices is acceptable. Please read below for details on what is accepted at events.

Attendees must wear masks on OSU's campus (until March 19, 2022) and refrain from attending the event if they have presented COVID symptoms or been exposed to someone with COVID in the past two weeks.

Proof of Being Fully Vaccinated
  • CDC vaccination record card, or a photocopy, photo, or mobile photo of the card.
  • Cardholders must be at least two weeks past receiving their last dose of the initial sequence of COVID-19 vaccine. (i.e.,second dose of Pfizer-BioNTech or Moderna vaccine OR first dose of Johnson & Johnson vaccine). Boosters are acceptable, too.
  • A valid driver’s license or another government-issued form of identification, such as a passport, must be presented with the vaccination record.
Proof of Negative COVID-19 Test
  • Negative COVID-19 test administered in the previous 3 days leading up to event date.
  • A test result must be in the form of written medical documentation (paper or electronic copy). The documentation must include test result; type of test; entity issuing the result; and specimen collection date.
  • The test must be a SARS-CoV-2 viral test (either nucleic acid amplification test [NAAT], including polymerase chain reaction [PCR], or antigen test) with Emergency Use Authorization or full approval from the U.S. Food and Drug Administration.
    • Home tests will not be accepted.
  • A valid driver’s license or another government-issued form of identification, such as a passport must be presented with the negative test record.

Please plan ahead to obtain your test results in time for the event. For local COVID testing in the Corvallis community, one recommended resource is WVT Laboratory (https://wvtlab.com/) in Corvallis.

Documentation of vaccine and/or negative COVID 19 test will be verified by an OSU staff member who is in compliance with OSU’s Vaccination Program but a copy will not be collected or maintained during the event.

Failure to produce sufficient proof of the full vaccination or negative COVID test as listed above will result in attendees being denied entrance to the event. The Office of Admissions offers numerous virtual presentations for those unable to attend.
