Registration and Enrolment

Requirements for Registration

You must meet all of the requirements for registration before you will be permitted to register in the program. The requirements for registration are:

  • Full immunization
  • Valid certificate in standard first aid and cardiopulmonary resuscitation (CPR) at the Basic Rescuer (C) level
  • Police Services Vulnerable Sector Screening
  • A signed Clinical Internship Catchment form
  • A completed WSIB form
  • Successful Safety Training results

Failure to meet the immunization, first aid, CPR, Police Services Vulnerable Sector Screening, and/or any other requirement(s) for registration may result in revocation of an offer of admission or cancellation of registration. If you have questions about the registration requirements, contact ptclined@utoronto.ca.

You must meet all registration requirements to participate in the program.

Please upload all the Registration Requirements through this Form by September 3, 2024.  Please note that you will need to have completed your UTORid registration and mail.utoronto.ca email to access this secure form.   You must be signed out of all other Outlook accounts before accessing the Form and log in with your UTORid account.

Immunization

You are required to be fully immunized before registering in the program. If you do not submit a completed immunization record, you will be suspended from clinical training until proper documentation is submitted. Returning students are required to submit an updated health form. Please read the instructions on the forms carefully for additional details. Keep a copy for your own records; clinical facilities may request a copy from you. In addition, some clinical facilities (e.g., long term care facilities) might have additional immunization requirements (e.g., three COVID-19 vaccines instead of 2).  This information will be made available for the students as we are informed. Download the Health Forms here.

Influenza

The Ministry of Health and the Department of Physical Therapy strongly recommend the influenza vaccine. Clinical facilities encourage all staff and students to receive the influenza vaccine. If there is an outbreak and you have not received the influenza vaccine, you may be redeployed by the site or, when appropriate, dismissed from the clinical site for the safety of yourself and patients. If you are dismissed from your clinical internship for more than one week, your situation will be reviewed by the EAC. See the Attendance Policy. If your internship is delayed for any reason, it may postpone your graduation date, eligibility dates to complete licensing exams, and may have implications for tuition and fee charges.

First Aid and CPR

You must provide a copy of a valid certificate in standard first aid and cardiopulmonary resuscitation (CPR) at the Basic Rescuer (C) level before being permitted to enroll in the MScPT program. You must complete this course between August 1 of the year you are accepted into the program and the first day of class, regardless of when your current certification expires. These requirements are set by our clinical partners and must be met so that you are able to participate in clinical practice. You will be required to complete your re-certification again at the start of year 2 of the program.

Police Services Vulnerable Sector Screening

As all physical therapy students undertake significant portions of their education in settings with exposure to vulnerable populations, you are required to complete and submit the results of a Vulnerable Sector Screening Check.

This process must be initiated through your local police service and must be completed prior to registration in the Physical Therapy program. Costs incurred will be at your own expense.

Police forces may use slightly different language to describe the various levels of record checks available. Your record check must include search results from local police records, Royal Canadian Mounted Police’s (RCMP’s) Canadian Police Information Centre and the Pardoned Sex Offender Records. This high level of record check is the one required by volunteer and other agencies who serve children and other vulnerable populations.

The Department of Physical Therapy also requests disclosure of any convictions in any jurisdiction and/or any findings of professional misconduct.

The Department of Physical Therapy reserves the right to revoke an offer of admission or cancel registration based on a review of this information.

If you have ever been convicted of a criminal offence for which you have not received a pardon, you are strongly urged to consult with your provincial College of Physiotherapists for advice about eventual eligibility for registration to practice physiotherapy.

Clinical Internship Catchment Acknowledgment Form

You must submit a signed copy of the Clinical Internship Catchment Acknowledgment Form before you can register in year 1 of the program.

WSIB Form

You must submit a signed copy of the WSIB Student Declaration of Understanding for Students on Program Related Placements

Safety Training

Regulation requires health and safety awareness training for every worker and supervisor under Ontario’s Occupational Health and Safety Act (OHSA). Under Bill 18, anyone completing an unpaid work term is considered an employee and needs to complete Health and Safety Education Modules. Please download the instructions for completed the mandatory Safety Training modules. You need to complete safety training modules and submit the following prior to being registered in year 1 of the program:

You may also be required to complete site-specific modules prior to starting an internship course.
