Reporting and learning systems for medication errors: the role of pharmacovigilance centres

Overview

This publication has been developed as part of the “Monitoring Medicines” project (http://www.monitoringmedicines.org/) funded by the Research Directorate of the European Union under its Seventh Framework Programme. It aims to enable readers to learn more about why adverse events occur with medicines, and what can be done to reduce patient deaths and negative health impacts arising from undetected problems with medicines safety globally. It provides a framework for advancing the application, coordination and optimal use of pharmacovigilance evidence, sharing that evidence and strengthening the links between national pharmacovigilance centres and other patient safety networks, to prevent medicines-related adverse events.
