“Rodent activity present as evidenced by rodent droppings found. 1. Approximately 20 rodent dropping observed on shelving, packaged single serve items, on the window sill, and beneath storage rack in dry storage room.”

“Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 1. One live small flying insect flying at the front counter.”

“Stop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Rice pudding 53F in front counter reach in display cooler, not prepared or portioned today and stored in unit overnight per operator. Observed ice buildup covering vents in unit. See Stop Sale. Repeat Violation.”
