Medication Errors: Beyond the 6 rights When identifying medication errors within medical records for a legal case, most of us begin with the 6 rights of medication administration and look for deviations from there. These rights include: Right Patient: Ensuring the medication is intended for the correct individual. Right Drug: Confirming the medication's accuracy and appropriateness for the patient's condition. Right Dose: Administering the precise amount of medication as prescribed. Right Route: Delivering the medication through the appropriate route (oral, intravenous, etc.). Right Time: Administering the medication at the ordered time. This also would encompass the indication for the medication. Right Documentation: Thoroughly recording all administration details for comprehensive patient records. Seems pretty straightforward at first glance, but a lot falls within this that can easily leave room for error. Did you know that: · Some medications can only be given in certain settings by RNs? · Some medications such as heparin can only have the dose adjusted by pharmacy, even in the intensive care unit (ICU)? · Almost all titratable (adjustable based on patient condition) medications can be given in critical care areas only? These areas include ER, ICU, OR and L&D. · Insulin drips require dosages to be adjusted based on blood sugars taken prior to each change? · Some meds will have specific parameters based on the patient’s assessment that the RN must be familiar with? I could go on!! I’ve seen the following in the hospital and in case reviews: · Medications given by RNs via a route that is outside of the RN scope of practice. The patient had a catastrophic outcome as a result of inappropriate monitoring and treatment of complications. · Medications given when the parameters set by pharmacy were not followed. For example, giving an oral blood pressure medication to a patient with blood pressure that is already low, resulting in a rapid response or code blue. · Weight-based medications are supposed to be programmed into pumps with the patient’s weight in kg. Errors in the weight entered can result in overdosing or underdosing, even if the medication dose is entered correctly. All of these errors were not written within the medical record. Each issue was identified because of familiarity with what was NOT there. This is the difference that a legal nurse consultant can make. 🌟 Exciting Milestone as a Clinical Pharmacist! 🌟 I still remember my first consultation as a Clinical Pharmacist, assisting a Doctor in Medicine. I must admit, it was quite nerve-racking. Covering a minor illness clinic, I thought, "Surely, he'll want a GP to assess him..." Well, to my surprise, it didn't go that way at all! It turned out to be one of the easiest consultations of the session. It felt more like a conversation with a friend.
