🏥 This Week in Med School 📚 I wrapped up the first month of the Medicine clerkship where I spent it at the Johns Hopkins Bayview Medical Center. This Monday I transition back to main Hopkins Hospital to be part of the Osler service, named after one of the four founders of Hopkins Medicine, Sir William Osler. Here are three of the salient takeaways from this past week: 1. *Be proactive with your health.* While the hospital is full of amazing life-saving interventions and caring providers, do whatever you can to keep yourself and your loved ones out of it. From the potential of nosocomial infections (e.g. hospital-acquired pneumonia, C. difficile) to iatrogenic issues (e.g. anemia from blood-draws), it's much better to be proactive with your health and reduce the chances of needing to be in a hospital, or if that can't be prevented to be able to recover faster. Check out Peter Attia's wonderful book, "Outlive," to learn more about Medicine 3.0 and how to prevent as much need for healthcare as you can. 2. *High-Value Medicine.* We had an excellent, engaging sessions with the Dean of the School of Medicine, Dr. Roy Ziegelstein, that brought to life the importance of practicing evidence-based and high-value medicine for both the individual patient (to avoid over-testing and medical error) and for the collective (to reduce healthcare costs and unnecessary utilization). We went into areas that aren't spoken about enough such as likelihood ratios from certain diagnostic tests and pre- and post-test probabilities. Will share a link to the overall topic in the description. 3. *7 H's and 5 T's Mnemonic.* These are ways to remember the reversible causes of asystole, pulseless electrical activity, ventricular fibrillation, and ventricular tachycardia: Hypovolemia, Hypoxia, Hydrogen ion excess (acidosis), Hypoglycemia, Hypokalemia, Hyperkalemia, Hypothermia, Tension pneumothorax, Tamponade – Cardiac, Toxins, Thrombosis (pulmonary embolus), Thrombosis (myocardial infarction). This is one of the first steps in finishing my master's in education. "It's probably just musculoskeletal." I heard it for years as a clinician — working in the musculoskeletal field. The quiet part is, "And so it's kind of out of my hands." "Musculoskeletal" is a big black box in medicine. many non-MSK clinicians don't feel comfortable dealing with it. ● HOW I BURIED RATS MANY YEARS AGO, TO PREPARE FOR MEDICAL SCHOOL 🙂 Many years ago, I used to literally walk by the road in search of de•ad rats 🐀. ☆☆☆ My friend Engr Kingsley Tasie had to call me one day to inform me of a big dead rat found in his compound. This was between 2008 & 2009. As of this time I hadn't been admitted into the university to study my dream course, medicine and surgery. I succeeded with 3 rats after much search. What was I doing with them? 🤔 I buried each seperately, and dug them up after some days when they had decayed just to extract their bones. Trust me, I did this when nobody was at home 😀. Some experiments require privacy. As someone who had just left secondary school a year ago [2007], and applying to become one of the best doctors in the society. I needed to get used to some important parts of the body before venturing into the system as a Medical student. So, while I was writing JOINT ADMISSIONS AND MATRICULATION BOARD , I had a container full of rat bones. As you know, rats and humans are mammals, so both have similar osteo-features. Inside the container were also some silica gel/desiccant [always found with new bags or shoes ] to absorb any humid and keep the container dry always. I got so used to the bones that I was familiar with various parts of the skeletal system as I often read with the bones beside me each time I studied. ● It was in 2010 that I got rid of them, when I became a Medical student in a private University. I remember bringing out my container in my room one day and my room mate was like, 'wetin you wan use this thing do, when you would be studying with the main human bones' soon? ~~~~~ Each time I remember those years of my life, I often smiled at my years of secret dedication and preparations for an unseen future. You can call it faith, motivation, dedication, planning, or even preparation, you are absolutely right ✅️ I may not be a medical doctor today, but sincerely, those little private moments of dedication has serious impacts in my life today. While working as a respiratory therapist, I gained experience in the cardiovascular intensive care unit, which led me to start my journey to becoming a perfusionist. Outside of my healthcare career, I enjoy world travel and immersing myself in different cultures. I live on a small farm in Georgia, where we raise highland cattle. ❓ AmSECT: Why did you choose to pursue a career in perfusion? 🙋Holly: My decade-long career as a respiratory therapist exposed me to the field of perfusion, which made it feel like a natural progression. I have always enjoyed working with different machines, so bypass and fluid dynamics intrigued me. I shadowed many different procedures to understand the role of the perfusionist in open heart surgery, which led me to purchase the Manual of Clinical Perfusion and apply to perfusion school. ❓ AmSECT: What is a perfusion-related topic that you think is indispensable to know for your perfusion career and why? 🙋Holly: One topic that is indispensable to the perfusion profession is the importance of evidence-based practice to improve current protocols and guidelines further. Through continued research, evidence-based medicine, and clinical registries, we can strive for continued advancements in the field of perfusion to optimize patient outcomes. As a perfusion student, it starts with understanding current perfusion practices. By understanding current practices within an institution, we can outline areas that need improvement and encourage research-driven protocols. An area of interest in which evidence-based practice could be applied to improve current practice is the use of serum and urine levels of MMP-2 and MMP-9 compared to creatinine levels for early indication of poor renal perfusion for patients undergoing bypass procedures. ❓ AmSECT: In reference to the question above, what question do you have about that topic that you’d like others' to input on? 🙋Holly: What are other areas of interest within the perfusion profession where evidence or further research is needed to improve a perfusion practice? #perfusion #studentfeature
