Standards for Transfers of Care (inpatient discharge, mental health discharge, emergency department discharge and outpatient letters) have been published for some years, but their implementation has been slow, resulting in the continuity of issues such as miscommunication between health and care staff in different settings and ineffective discharges. PRSB was commissioned to review the reasons for limited progress on e-discharge and to make recommendations on how this could be improved for better continuity and integration of health and care services. The unsung heroes in our patients' recovery process, especially in ICUs, family caregivers provide essential psychological comfort and, importantly, contribute to post-ICU syndrome symptom reduction. This can enhance the quality of life for our patients, improve survival rates, but beyond this — improve our business in more ways than one. Family caregivers are often our key liaisons, acting as patient advocates and helping to relay specific symptoms or limitations to us, the therapists. They can also facilitate adherence to at-home exercises, boosting treatment outcomes. This underlines the immense benefit in nurturing relationships with these caregivers, leading not just to enhanced patient satisfaction, but also fruitful word-of-mouth referrals. How can text messaging improve patient well-being? We partnered with Ventura County to provide SMS surveys at regular intervals to assess mood and wellness of individuals leaving psychiatric inpatient facilities. Not only did patients rate the program highly, but the automated SMS messages saved Ventura County staff hours of tracking down clients by making repeated outbound phone attempts.
