Standards for Transfers of Care (inpatient discharge, mental health discharge, emergency department discharge and outpatient letters) have been published for some years, but their implementation has been slow, resulting in the continuity of issues such as miscommunication between health and care staff in different settings and ineffective discharges. PRSB was commissioned to review the reasons for limited progress on e-discharge and to make recommendations on how this could be improved for better continuity and integration of health and care services. How can text messaging improve patient well-being? We partnered with Ventura County to provide SMS surveys at regular intervals to assess mood and wellness of individuals leaving psychiatric inpatient facilities. Not only did patients rate the program highly, but the automated SMS messages saved Ventura County staff hours of tracking down clients by making repeated outbound phone attempts.
