Digital health startups like Foodsmart, grocers like Albertsons and Kroger, delivery fulfillment partners like Instacart, and even government agencies like the U.S. Department of Health and Human Services (HHS) are ramping up food as medicine programs, which combine features like nutritional guidance and grocery delivery to support consumer health. In the past six months, at least 17 digitally-enabled food as medicine programs have launched across the country. Program configurations vary by who initiates (the program onboarding), who fulfills (the food delivery), and who pays (for the service and food). Here’s how recently-announced food as medicine programs are operationalized and funded: • Generally in a food as medicine program, the service flow doesn’t match the direction of cash flow. For example, a clinician might initiate a consumer’s food as medicine consultation, even if the program is paid for by the health plan. • Health plans are the main funders of food as medicine programs, covering costs like nutrition consultations and grocery stipends. In particular, Medicare Advantage and Medicaid plans are ramping up food as medicine programs as supplemental benefits. • Even with plans leading the funding charge, startups and fulfillment partners often accept multiple forms of payment in addition to plan benefits, including SNAP, FSA/HSA, and out-of-pocket spend. This enables them to scale with multiple channel partners and flex if plans or funding policies change. Food as medicine innovation is still early on the innovation curve. Retail and health care. Walmart just announced they're shutting down all 51 of their (what was supposed to be rapidly expanding) health centers. On April 16th, Shoppers Drug Mart sold their Health Clinic to another health care provider (WELL). Although not a perfect match, having a Dr adjacent to a pharmacy seems to me like a really effective way to offer a meaningful consumer benefit - get clear and direct access to the products that will improve your healthcare outcome. Diabetics and their needs seems like a good example. Apparently the economics weren't working. There was either enough evidence that the long term bet couldn't work, or in this current economic environment w expensive money, inflation, etc, focus on core is more important than long term bets (that aren't technology based).

This is not an altruistic effort and a healthy margin will be required to make it worth the effort to keep these going in the long run.” I wrote that on March 11, 2020. The long run turned out to be about four years. To many of us with long careers working in healthcare, Walmart’s announcement earlier this week was shocking, but hardly surprising. Primary care is a challenging fit for retailers who prefer high-margins and have historically been able to tap into a readily available, low-skilled workforce to meet labor needs. Traditional health providers have struggled with primary care profitability for at least the last two decades. 👇 𝐓𝐡𝐞 𝐛𝐨𝐭𝐭𝐨𝐦 𝐥𝐢𝐧𝐞: This is no time for health systems to back away from innovation or efforts to improve the patient experience. Patients are losing access to primary care at time when that is already a huge societal problem. Walmart Health closing just exacerbates the issue. There are no winners in that outcome. OK - let's see if I have this right. Walmart can't make a go of primary care. Shoppers Drug Mart recently sold the Health Clinic by Shoppers™, for much the same reason, and I understand Amazon is struggling with their Health offerings. And in #Canada we expect #familyphysicians with NO business experience to make a living in the current #healthcare system???
