How did we improve our Hospice Visits Last Days of Life (HVLDL) score by 61%? Using the power of #PredictiveAnalytics, we developed a machine learning algorithm that identifies patients who are likely to pass away within the next seven days. This data is compiled into a dashboard, allowing hospice teams to schedule the correct number of visits for these patients. Read about how we leveraged predictive analytics for our end-of-life services and how we can help your organization as well. #AIinHealthCare #Hospice

It is interesting to note the similarity in challenges different VBHC implementors are facing, including 'upcoding', and what interoperability can do to better support POC decision-making. We develop business rules with providers to mitigate such issues, but it is never-ending on how many other rules one has to keep creating-usually after a pothole. #vbhc #momcare Daniel Kapitan

Today, we published a peer-reviewed study in Nature Scientific Reports demonstrating that Waymark Signal — our proprietary machine learning technology — is 90% accurate in predicting avoidable emergency room (ER) and hospital visits for people receiving Medicaid. Drawing on CMS data from 26 states, the study found that Waymark Signal is 3x better at identifying at-risk patients and 10x better at predicting costs compared to conventional risk models in Medicaid. These findings highlight the potential for tools like Waymark Signal to help care teams more accurately identify at-risk patients — and ultimately deliver more equitable and effective care for underserved Medicaid populations. Day 1 of #vive2024 and I want you to go check-out Oshi Health. I had the pleasure of speaking with Randy Forman on Highway to Health recently, and I learned so much about the patient community suffering from chronic GI conditions. 25-33% of our population has a diagnosed digestive issue -- 2-3x the prevalence of diabetes. If that's not eye-opening enough -- upwards of 60-70% of our population deals with some sort of digestive issue -- often undiagnosed or less severe. The problem is -- coding methodologies are necessarily designed to document the commonality of these conditions, so our payer and provider communities have a hard time quantifying the percentage of their population suffering. Plus -- because of reimbursement structures -- the necessary care and coordination was largely unknown or unavailable to the majority of patients. On Highway to Health, we celebrate people and organizations that are committed to improving access, experience and outcomes for our industry -- and Oshi / Randy are perfect examples. The Office for Civil Rights and Centers for Medicare & Medicaid Services issued an unpublished version of a final rule under Section 1557 of the Affordable Care Act that, among other things, imposes new requirements of covered entities around their use of #ArtificialIntelligence tools in clinical decision-making. The final rule will directly affect all recipients of federal financial assistance, including health care providers, and serves as an important next step as federal agencies continue to leverage existing authorities to regulate certain uses of AI in health care. Please check out our review of the new rule here:

Strategic Minded Consulting- Mid-Rev Cycle Staffing Strategist - Case Management and Utilization Management Optimization - Helping to Ensure Quality Improvement and Increased Rev Cycle Efficiencies
Check out this RAC Monitor article by Ronald Hirsch, MD, FACP, CHCQM, CHRI, APCA-C. I have said it before, and I will say it again. In order to get the best outcomes for your patients and facility you MUST have the Human Element present. People respond to people, and it is people who can think creatively to solve problems. AI, while useful from a predictive analytics standpoint will not be able to understand the complexities of one-off situations and why outliers should be treated differently. At least in the near term. Having appropriate levels of staff in paramount for Population Health, Financial Solvency of your facility, and it is a Goodwill to your community. If Hospital Executives continue to apply a linear approach to a nonlinear problem (Staffing), we will never get off the hamster wheel, and more facilities will close/ be acquired by big corporate operations, which will cause communities (especially rural) to suffer. Who wants to be a hero to their boss and community? #Healthcare #CaseManagement #SocialWork #staffingsolutions https://lnkd.in/em7ZQmmf
