Success for ACOs and other value-based care arrangements depends on right-sizing care and keeping the locus of care in the community. According to new research, home-visit based primary care led by advanced health care practitioners offering primary care is very successful at that for the neediest and frailest patients. Such a program at Memorial Care Medical Group in California decreased costs by 21.4% while reducing inpatient and emergency department visits as well as 30-day readmissions. A great model to follow.
