Success for ACOs and other value-based care arrangements depends on right-sizing care and keeping the locus of care in the community. According to new research, home-visit based primary care led by advanced health care practitioners offering primary care is very successful at that for the neediest and frailest patients. Such a program at Memorial Care Medical Group in California decreased costs by 21.4% while reducing inpatient and emergency department visits as well as 30-day readmissions. A great model to follow. Too many transformations stop at the operating model and business model, but the real magic happens when the way we work transforms. That requires a vital, engaged, and skills growth-mindset based workforce. As I read this Becker’s article with prominent CEOs talking about their transformations, health systems must focus on transforming “how employees and clinicians work” in addition to the changes in the business model and the physical bricks and mortar moving to ambulatory/outpatient settings.
