Someone I meet: "What kind of work do you do?" Me: "I work with Community Health Centers to ________________" 🚀 Assist in transforming the practice to a Patient Centered Medical Home (PCMH) 🖇 Analysis of policies & processes to identify opportunities for improvement & assist in the development of best practices 💲 Assist practices in the successful implementation of Quality Programs such as a Medicare Wellness Visits, Transitional Care Management, etc. Most often (unless I am talking with a fellow FQHC'er 😉) the next question goes something like this.... Closing the gaps in our health systems requires bold innovation and community-focused outreach strategy that address health disparities at their root. Bold, community-focused, and strategic is exactly what describes NYC Mayor's Public Engagement Unit's parentship with NYC Health + Hospitals's NYC Care program. Check out PEU's Deputy Director Isolina De La Cruz & NYC Care Executive Director Jonathan Jiménez Pérez in this great coverage by El Diario NY that details NYC's efforts to expand health cares to New Yorkers otherwise ineligible or unable to afford health care. Health disparities have the greatest impact on marginalized groups, like the Latinx community here in NYC, so as Latino, it's great to see two Latinx leaders leading the effort to close health disparities in NYC, and the leading Latinx newspaper in NYC spearheading the coverage that informs NYers of the support their city offers them. #nyc #nyccare #health #access

CHWs are vital for universal health coverage (UHC)! Undeniable evidence shows that CHWs deliver primary health care in a way that improves access, increases equity, and saves lives. That’s why at #UNGA78 we’re advocating for professional CHWs (#proCHWs) who are salaried, skilled, supervised, and supplied to be the norm worldwide. Join us as we work towards #healthforall. Our long-term partnership with Care Ring is rooted in community voice and building relationships that optimize true care for a community - and not just transactional services. This week's article from Ryan Pitkin at The QC Nerve highlights how this work influenced locations and offerings for Care Ring's mobile health clinic - The Bridge - and in turn, more people receiving the healthcare they need. We're so inspired by the way our teammate Helen Hope Kimbrough builds trust in communities by listening and being proximate to the issues neighbors face - including, in this case, what it might take for an individual to access the clinic and making that walk herself. As she says in the article, “I wanted to see what it would be like for a mom to walk with two kids, or for a person who does not speak English or a second language, how would they navigate that?” she explained. “I also wanted to see what type of things would be needed for a person who may be disabled. And so along that walk, I got a chance to kind of embody some of that, because I feel like some of this work people come to with a bias. So how do we pull back all of that so that we can literally look to see the neighborhood — what’s going on, the people, the bus routes, all of those things?” You can read the article here: https://lnkd.in/eAXsEGbd For more about what Next Stage believes about Community Voice, read our recent report, "Inside Out: The Case for Community Voice:" https://lnkd.in/efvfu-4R

This…bottom line: HCs save the system and taxpayers BILLIONS by lowering the cost of care, avoiding costly ER visits and managing chronic illness. To support community health centers for the long term, we must rethink and strengthen the financial foundations of the health center ecosystem. During testimony in support of the Senate’s CHCF reauthorization bill, Advocates for Community Health CEO Amanda Pears Kelly cited data stating that health centers save $24 billion in health spending per year through more cost-effective care. Let’s discuss community partnerships for a moment. 🤔 With healthcare, our seniors deserve a variation of transitional care options. You run into moments where the services you offer does not fit the need of the client. To better serve them, other options should be mentioned!! This year, two topics will dominate the health discourse at the upcoming #UNGA78: universal health coverage and pandemic prevention, preparedness and response. The two topics are normally treated in siloes but find common ground when considering the need for “equitable, people-centered and community-based primary care”. This can only be achieved through the integration of community health - and paid, professional, and supported community health workers into the formal health system. Community Health Impact Coalition, mothers2mothers, Integrate Health, Muso, VillageReach and many others will be taking community health and #healthforall across the ocean to New York… to change the water. Not only are these community health centers are in need of funding but are in desperate need of a makeover on how their operations function. From a consultant's perspective, there's been plenty of times where we've seen workflows that constantly omits money on the table for every patient lifecycle within these respective facilities. More importantly, these deficiencies drastically affects the quality of services that the community receives thus creating a huge reduction with patient access. Adding more money to a system without the proper infrastructure is equivalent to filling water into a leaky bucket. #fqhcs #communityhealthcenters #communityhealth #patientaccess #healthcareconsulting https://lnkd.in/eqeKDDUr
