Infectious disease specialist Leila Hojat, MD, arrived at UH Cleveland Medical Center in September 2019 to lead the hospital’s antibiotic stewardship program. Advocating for the wise use of antibiotics has long been her passion. Her goal, she says, is to help clinicians make sense of the burgeoning number of antibiotic clinical trials and guide them in making good prescribing decisions for their patients -- decisions that won’t promote antibiotic resistance. But something happened just a few short months into implementing this vision: COVID-19 descended, which required the immediate involvement and expertise of every infectious disease specialist in the country. Now that the worst of the pandemic is behind us, Dr. Hojat and her antibiotic stewardship team have received an important accolade: Under her guidance, University Hospitals has earned the prestigious Infectious Diseases Society of America Antimicrobial Stewardship Centers of Excellence designation. Do you remember having an infection that only got sorted with an antibiotic? You know the relief of getting better? 💪💊🛑 Without antibiotics, infections are mostly likely lethal or cause disability. 💊 Medical care depends on #antibiotics and antifungals working. Microorganisms adapt and evolve very fast (they have been here on Earth 🌎 for billions of years, so believe me, their genetic material keeps all the secrets of survival and adaptation). As a consequence, they have an incredible capacity to become resistant to one or multiple antibiotics. Pollution we caused in the environment has accelerated the speed at which microorganisms adapt. We basically created a super-duper environmental gym where they train their "genetic muscles" - allow me the analogy here - with the most amazing chemical mixtures, and in the environment we created, they learn much better how to survive and adapt. So, when they infect a person who has some sort of weakness, has undergone surgical treatment, or a newborn whose immune defenses are building up, you know what happens; in a matter of hours or days, the result is often very sad. We need to improve our environmental quality, infection control measures, rates of vaccinations, and definitely create incentives to fund, support, and give market resilience for the pharmaceutical industry to develop new antibiotics 🌐💉 Let me share with you a relevant event on #AMR. New publication within The Lancet Infectious Diseases: How to tailor recommendations on the treatment of multi-drug resistant Gram-negative infections at country level integrating antibiotic stewardship principles within the GRADE-ADOLOPMENT framework. In 2021 the Italian Medicine Agency (AIFA Agenzia Italiana del farmaco), appointed a working group to support its main scientific committee to optimise the use of antibiotics in Italy. This article describes the process of the working group on the development of international evidence-based guidance documents on treating infections caused by multidrug resistant bacteria (MDB) by using the GRADE-ADOLOPMENT Framework methodology and integrating antibiotic stewardship principles. New study alert! Addressing the looming threat of antimicrobial resistance, rapid point-of-care tests for infection (POCTs) emerge as a promising solution to curb overprescribing. A new vignette-based study, led by @Suveer Singh and @Martine Nurek of Imperial College London, confirms that negative POCT results can significantly sway ICU doctors to stop antibiotic treatment. However, factors like the doctor’s perceived need for antibiotics and their interest in utilising POCTs also played an important role. 🌟 This study signposts important clinical, cognitive, and behavioural factors that influence antibiotic prescribing in ICU, paving the way for smarter, more effective healthcare strategies. In Norway we are pleased that our most prescribed antibiotic for treating infections is penicillin v (phenoxymethylpenicillin), first marketed in the 1950s. The main indication for narrow-spectrum penicillins in primary care is respiratory tract infections. Therefore, we monitor penicillin-resistant pneumococci. As you can see from the graph showing the number of reported bacterial isolates with penicillin-resistant pneumococci in Norway from 2012 to 2023, the numbers remain very small. There was a small “peak” in 2016 (less than 30 isolates), and only six people with penicillin-resistant pneumococci in 2023. Older antibiotics like phenoxymethylpenicillin still work, and we need to maintain reliable access to them. Important for patients. Important for hindering antibiotic resistance #amr. I'm very pleased that in EU-JAMRAI we are working, in collaboration with our national and European colleagues, to strengthen access to critically important antibiotics like phenoxymethylpenicillin. The webinar was expertly organized by International Society of Antimicrobial Chemotherapy (ISAC) Academy, led by the esteemed Prof. Geoff Coombs, President of International Society of Antimicrobial Chemotherapy (ISAC), and Asia-Pacific Society of Clinical Microbiology and Infection (APSCMI). The topic of the webinar was timely and critical: "One Health Aspects of #AMR & Infections in Asia". 2023: The year that was I’ve written, edited, proofread and project-managed more than a million words in 2023 across websites, articles, grants, reports, app content and books—but there have been three highlights, and here’s why: ⭐️ Interesting—project managing and editing AURA2023 allowed me to see the trends in bacterial and fungal infection resistances and antibiotic usage across Australia, and to communicate these widely. Our healthcare and aged care settings hold some of the most vulnerable members of our society. Knowledge is power: data and surveillance are the keys to preventing infection and having drugs that can effectively treat infection. 💡 Inspiring—writing case studies for a Sydney University/Deloitte project that measured the wider impacts of research required me to interview Professor Weiss, whose research fundamentally changes the current approaches to wound healing and treating tissue damage, such as after a heart attack. Interviewing him was just awe-inspiring. 🫶 Impactful—writing healthcare professional and consumer books for Indigenous Australians on maintaining a strong and healthy heart for the Heart Foundation. Coronary heart disease is Australia’s number one killer, taking 1 Australian life every 12 minutes. I hope that these resources will help prevent this potentially silent killer. 🥂Here's to a healthy, meaningful and peaceful 2024.
