Dismissive treatment of a patient's symptoms, commonly referred to as medical gaslighting, can occur based on factors like gender, age, race, or ethnicity, leading to patients not being taken seriously or their symptoms being downplayed. Z. Colette Edwards, MD MBA emphasizes the systemic challenges within healthcare that contribute to poor care coordination and patient advocacy in Episode 15 of The Desperate For a Diagnosis Podcast. The increasing complexity of the healthcare system, coupled with technology that does not always facilitate efficient communication, further exacerbates the problem. Physician shortages and the lack of interdepartmental collaboration can result in fragmented care. Dr. Edwards also highlights the importance of cohesive, physician-led group practices that ensure comprehensive care for patients. She emphasizes the need for patients to advocate for better healthcare systems that prioritize effective communication, coordination, and accessibility to physicians within networks. It really is not that complicated. Let's have an honest dialogue. 1) Academic medical centers (especially "big name ones") have dismissed Family Medicine since the federal government introduced it in the early 70s -- steering students away from it but misleading legislators with their annual reports of X% of graduates entering "primary care" (IM & Peds resident positions -- yet 80-90% go on for subspecialty fellowships). 2) talk about the elephant in the room -- physicians who practice part-time (male & female since many are married to each other and/or have financial flexibility) or don't practice at all. 3) let's not forget the growing group who do NOT accept the longstanding principle of "altruism" in medical practice. Their mantra is "I'll fit my practice into my lifestyle, not I'll fit my lifestyle into my practice because I have an obligation to my patients & colleagues." 4) let's not forget the "big lie" oops -- the big wish -- we are admitting students who will practice in our state, our underserved areas, our minority populations, fill in your favorite "need" here & insert it into your school's mission statement. 5) the geographic maldistribtion of physicians that people refuse to tackle with "required assignments"

The physician advisor had to argue for the medical necessity of the hospital stay, focusing on the treatment of pneumonia. - The second case discusses a 74-year-old female with hyponatremia and dizziness. The physician advisor had to argue for the inpatient level of care based on the severity of illness, utilization of resources, and medical necessity.
