i am afraid i don’t agree Oleksandr Babliak. with an head camera, transaxillary allows assistants to help and learn very well. I never feel alone! At least in my hands, compared to RAT (that we have abandoned), trans-axillary is easier and faster and, on top of that , it allows a much better exposition and operator friendly working conditions. also debetable the notion that transaxillary is for the beginners while endoscopic for the experts. in fact while direct view transax may certainly help beginners to start a MIS program (you see it - you do it), with intermediate surgeons patient’s selection will be further minimised with interventions getting faster and faster. the experts ? will do 110% MIS including double/triple combined valve interventions, AFib and LAAM. The result is that with transaxillary - that enhances surgical exposition without increasing technical complexity - MIS can expand with all kind of surgeons, not just the beginners. longer thoracotomy? …the SINGLE INVISIBLE INCISION is unsurpassable by all other mini AVR approaches. Including presenting surgeon Joerg Kempfert One point for surgeons who wish to start this pathway: The more you are lateral, the more you will be alone (as a surgeon) during valve replacement with exposure, excision or suturing. The first assistant will be able to help in the RAT setting, but will not be able to help in endoscopic or axillary approach. This point is important, because surgeons are used to have the assistant help in median sternotomy. So, going for them to minimally invasive approaches this fact should be kept in mind.
