Things have changed a decent amount since your early CareMore Health hospital at home program days 10 years ago prior to the CMS waiver. I’m not sure where the toxic positivity comment is coming from (is it the tech vendors? The consultants?) but we have a healthy appreciation for the value that HAH creates for patients and families and payers and feel that this is an important tool for many patients with reasonable guardrails around patient optionality and caregiver support and health equity. And no this is not about providing less care for the same price. This is about providing more and better care at the same acute cost and lower 30 day costs. There are a number of concerning evidence free statements in this article pertaining to “HAH is just pushing nursing work to family.” I’m sure you didn’t do that in your early Caremore program and we certainly don’t do that in our hah program. We evaluate all patients and family caregivers for interest and often family caregivers are delighted to have HAH. If caregivers are not delighted to have us and either the patient or the caregiver prefers to stay in brick and mortar they simply decline HAH. And that is ok! Also we offer HHAs routinely as a matter of care to support patient ADLs! And yes of course we should all absolutely and categorically want fewer hospitalizations for heart failure. Just like we should all want fewer patients coded with high level HCC codes for heart failure. Prevention and quality are still the key!

Initiation of #antihypertensive medication is associated with elevated risks of #fractures and falls older long-term nursing home residents. These risks are higher among patients with dementia, higher baseline blood pressures values, and no recent antihypertensive medication use. Over the past two weeks, I have reviewed two cases related to nasogastric tube placement, which resulted in serious complications. In the first case, a woman suffering from dementia passed away due to pneumothorax that occurred following the insertion of the nasogastric tube. The second case involved a young man who required multiple attempts at inserting the nasogastric tube, resulting in an injury to his maxillary sinus. As a medical professional, these incidents have highlighted the importance of educating others about the proper placement and management of nasogastric tubes. Therefore, I plan to share a series of informative posts over the next week to raise awareness about the potential risks associated with nasogastric tubes and the importance of correct placement. In this series, I will cover everything from the basics of nasogastric tubes to the intricacies of their safe and effective management. I will provide information on preventive measures that can be taken to avoid the grave complications that can occur during nasogastric tube placement. Nasogastric tube placement is an extremely common procedure performed in multiple healthcare settings, and it is essential to foster a dialogue on best practices to enhance patient safety.
