There seems to be so many barriers to patients starting Cardiac Rehab programs and committing to them. While I have seen the industry come a long way in some areas, such as automatic referrals and physician awareness, there are still many barriers. On a daily basis, I see a drastic need for transportation, patients in rural areas over an hour from our hospital having no options, the distance driving 2 hours every day three times a week is a huge burden. Affordability is an issue as patients still either drop out or never start due to high co-pays. In general, I see that patients don't quite understand the importance. They think it's just an exercise program and they don't want to participate or they will only participate if they hear from their doctor to make sure he/she wants them to participate, even though they are the ones who referred them. This bipartisan legislation would add a permanent, Medicare Economic Index-based inflationary update to the Medicare Physician Fee Schedule. Providers who bill under the fee schedule do not automatically receive an annual inflationary update. Without an inflation-based update, the gap between frozen fee schedule payment rates and rising practice costs due to inflation continues to widen considerably. The increasing discrepancy between what it costs to run a practice and actual payment, combined with the administrative and financial burden of participating in Medicare, is contributing to provider burnout, decreasing patient access to care, and is unsustainable. H.R. 2474 would change this flawed system to allow providers who bill under the fee schedule to receive inflationary updates, based on the Medicare Economic Index, similar to those provided to other payment systems under Medicare. While there are many other concerns and reforms to the Medicare fee schedule that still need to be addressed, this legislation is a great first step by providing stability to the Medicare fee schedule that will help ensure patient access to care. My goal as a medical navigator and patient advocate is to help people with complicated healthcare problems get the best care possible. Our rehabilitation system for people with severe injuries such as traumatic brain injury or spinal cord injury now provides very short lengths of stay in an inpatient acute rehabilitation facility. As a result, people go too early to sub-acute rehab in a nursing home, where there is a very different level of expertise and equipment.
