The issue of overcrowding in Emergency Departments is significant, and solutions seem scarce. Patients often endure long waits, whether they're awaiting an evaluation by a healthcare provider, discharge, or admission. The strategies mentioned in the article, devised by physicians and healthcare systems, are commendable. However, introducing trained patient advocates into the Emergency Department could further enhance patient flow, address patient inquiries and worries, and free up clinical staff to focus on essential medical tasks, thereby optimizing operations. I am looking forward to speak at HSJ's Patient Safety Congress on 18-19 September 2023, in Manchester. Making Families Count is speaking on Day 1 of the Congress, at 12.25pm. The title of our presentation is: "All families want is someone to blame - Perceptions of working with harmed families and patients versus the reality and benefits" This session will examine why the idea that families are only looking to apportion blame following a serious incident is a myth which is getting in the way of patient safety and embedding learning. We will look at: The reality of what families go through when bereaved through serious healthcare incidents. Spotlight what families most want from an investigation. Look at what healthcare professionals gain by working positively with patients and families. Successful projects created by families co-working with healthcare professionals – spotlighting the MFC project “Life beyond the Cubicle”. Featuring powerful personal stories by bereaved family members from MFC, including films and audios designed to support health professionals reflect on their practice.

Your systems and processes shouldn't be setting people up for failure. We have known for 20 years that systems within care settings need to account for human error. In a blog post we examine the RaDonda Vaught case, a former nurse at Vanderbilt University Medical Center, who mistakenly administered a powerful paralytic instead of a sedative with a similar name. The patient, a 75-year-old woman, died. Vaught admitted the error and acknowledged there were warning signs that she missed during the incident. She was cleared by the Tennessee Board of Nursing but criminally charged by local authorities. Our take: existing systems could be doing more to prevent errors and ease workloads. Rather than spending time and effort on convicting someone for a mistake, wouldn’t resources be better spent on preventative measures within healthcare systems?
