California's 1.1 million undocumented workers make up 6% of its labor force, according to UC Merced's Community and Labor Center. In 2019, undocumented immigrants paid an estimated $3.7 billion in state and local taxes, according to USC's California Immigrant Data Portal. This does not account for their purchasing power, which helps sustain local, state, and national economies. Based on updated Medicaid data, the federal government spent $974 million in total in 2016 on life-saving and emergency services for undocumented immigrants. This amounts to 0.2% of Medicaid spending (or roughly $565 billion) and 0.03% of all national health spending (or roughly $3.3 trillion) in that same year. In 2016, California spent $174 million on emergency department (ED) services for undocumented individuals representing .6% of California Medicaid expenditures that year, which totaled approximately $28 billion. The estimated cost of unnecessary ED care has nearly doubled from a 2010 estimate. That because ED visits are, on average, 10x more expensive than an urgent care or preventative care visit. California’s move to expand access to healthcare is not only a commitment to human rights, but also a sensible economic move because research indicates that undocumented immigrants are a robust and productive segment in US and state economies, but, additionally, early access to healthcare improves health outcomes in the mid-long-term, which reduces morbidity rates and ultimately creates cost-savings both to consumers and taxpayers.

The U.S. uninsured rate reached a historic low in 2023, but millions of people still lack comprehensive health coverage — including a disproportionate number of immigrants. On the blog, Georgetown University’s Justin Giovannelli and Rachel Schwab say that while undocumented immigrants have limited health insurance options, some states are taking steps to expand coverage. For instance: ☂️ Colorado is covering a limited number of low-income undocumented immigrants through a state-run program with no monthly premiums and substantial cost-sharing assistance. 📝 Washington State is using a federal waiver to allow undocumented immigrants to enroll through its Affordable Care Act marketplace, with state-provided premium assistance. 💵 Minnesota and New York are using state dollars to expand access via their Basic Health Programs. The authors point out, however, that these efforts have only a modest reach. A comprehensive solution, they say, will likely require changes to federal law.
