Imagine the heart-wrenching task of informing a dog owner that their beloved pet has carcinomatosis—a diagnosis that often carries a bleak prognosis. However, it is not always an immediate death sentence. Amidst this devastating news, there are treatment options capable of enhancing the quality of life and extending survival, offering precious moments together during this challenging period. For a long time, I have always recommended intracavitary chemotherapy and intravenous chemotherapy. However, based on a paper that was officially published online this month by Hicks et al. in Veterinary Comparative Oncology, oral Palladia is an alternative that could be considered and can help extend survival in some dogs with this devastating cancer. However, it's important for pet owners to understand that there is no cure. Despite the varying approaches to treatment, official guidelines have been missing—until now! 🎯 Objectives: A team of board-certified specialists (Karen Munana, Ned Patterson, Simon Platt, Holger Volk) lead by Marios Charalambous joined forces to establish evidence-based guidelines for managing seizures in dogs and cats. Their mission: to provide a unified approach to tackle these critical situations. 🐾 What's Inside: The panel assessed peer-reviewed literature, incorporating veterinary pharmacokinetic studies, basic research, and human medicine to formulate consensus clinical recommendations. Meet Mr Mole 😍 Mr Mole was referred to us by Laura Korenchy from James Specialist Veterinary Cardiology for cardioversion of his atrial fibrillation. Atrial fibrillation, like in people, is the most common arrhythmia that affects dogs. In certain cases we can use cardioversion to convert the atrial fibrillation into a normal sinus rhythm. Indeed, for people cardioversion is often the treatment of choice for atrial fibrillation. Mr Mole had already received extensive evaluation by Laura. He was admitted and following some further blood work he was anaesthetised by one of our dedicated anaesthetists. The second attempt at cardioversion was successful in returning Mr Mole to a sinus rhythm, meaning that he could stop one of his anti-arrhythmic medications. Here he is following recovery from anaesthesia. It is unclear at this stage whether sinus rhythm will be maintained in Mr Mole, but Laura and his owners will keep a close eye on him. However, it can be challenging to know when to worry. In this course, we will cover a wide range of cardiac conundrums from those puppy and kitten murmurs to the collapsing patient with ventricular tachycardia. We will discuss new treatments and management strategies for congestive heart failure. There is a special session covering the principles of anaesthesia in animals with heart disease, presented by Matt Gurney (EBVS and RCVS Recognised Specialist in Veterinary Anaesthesia and Analgesia) from Zero Pain Philosophy. vtx:think We continue our internal medicine series with 'U' for Ursodeoxycholic acid (UDCA). Ursodeoxycholic acid (UDCA) is a hydrophilic bile acid that is believed to have several beneficial properties: UDCA may displace more toxic hydrophobic bile acids from the circulating pool, it has a choleretic effect which increases the excretion of endogenous toxins in the bile, it has a cytoprotective effect by inhibiting hepatocyte apoptosis, and immunomodulatory effects, such as the suppression of interleukin-2 expression. It might also be my favourite word in the veterinary dictionary! Because of its choleretic effect and the displacement of more toxic hydrophobic bile acids, there is a rationale behind using this drug in dogs and cats with intrahepatic and extrahepatic cholestasis, such as cholangitis in cats. The use of UDCA in dogs and cats with complete bile duct obstruction is controversial as some clinicians are concerned about the possibility of it increasing the probability of gallbladder rupture. Other clinicians feel comfortable using UDCA in this situation and studies in rats where bile duct ligation was performed indicate that it has a beneficial effect on markers of oxidative stress and apoptosis. However, it is important to note that surgical intervention is indicated in most dogs or cats with complete bile duct obstruction. Where the patient has no or mild clinical signs, UDCA is sometimes used as part of the medical management of non-obstructed, canine gallbladder mucoceles. Furthermore, due to its purported immunomodulatory and antiapoptotic effects there is a theoretical reason to use UDCA in dogs with chronic hepatitis although its efficacy in this condition has not been critically evaluated. Lawsonia intracellularis causes proliferative enteropathy (PE) in horses and in various other species, such as pigs. Affected horses often present to a clinic with diarrhoea along with other clinical signs. This article discusses the treatment and nursing of a 6-month-old Thoroughbred foal with equine proliferative enteropathy, including nutrition, fluid therapy, medications and general nursing care. Samantha demonstrates how having a clear nursing care plan ensures standardised, effective treatment and also allows for evaluation and changes to be made dependant on the individual patient. 🐈 Danielle Barnett shares her thoughts on how we should be advising clients on their choice of cat litter. When inappropriate urination has been cited as a major cause of rehoming cats, and once other causes have been evaluated, the choice of cat litter may be a significant part of the cat urinating in other places apart from the litter tray. Veterinary nurses can give guidance on the range of cat litter available enabling the owner to make a choice based on their cat’s preferences.
