Long read: for NHS costing teams, balancing national cost collection processes and ensuring data is used to provide local benefits is an ongoing resource challenge. Recently, the Healthcare Costing for Value Institute reviewed 10 tests that need to be met to achieve good #NHScosting. The tests are as follows: Test 1: Cost data is regularly used in decision-making to drive improvements in value in the NHS. Costing supports the future information requirements of the NHS. Test 2: Cost data from the national cost collection is fed back in a timely manner to local health economies in a way that supports them to improve value. Test 3: There are a set of national costing standards to ensure a consistent approach to patient-level costing. Test 4: The standards are proportionate, achievable, and easy to understand. Test 5: There is a single version of cost data that can be used locally and nationally." Test 6: Local leaders ensure that there are robust data governance processes in place for the non-financial data required for costing. Test 7: The role of cost accountants includes creating cost data and supporting their local health services to use the data to improve value. Test 8: Local and national costing teams are adequately resourced with staff who have the right skills, knowledge and experience. Test 9: Decisions made about changes to the national costing approach are transparent. Test 10: The development of the national costing approach is done in close partnership with local teams. For a number of years, NHS costing practitioners have had to balance national cost collections with using the data to harness benefits locally. These tests were first outlined by the Institute in 2021 but remain a valid overview or what is required to achieve good costing. With the future of NHS costing still uncertain, they are useful for assessing expected outcomes from the NHSE current future of costing project and as a way to assess plans to move to the federated data platform for data submissions. Is there something wrong with asking Value Analysis (VA) to focus on cost reduction, & quality of care by selecting products that perform best based on “EVIDENCE”? Asking the AHVAP to address their Board representation ( 1 GPO (Premier) has 5 employees, and 2 member hospitals on a 9 person Board) conflict of interest? The GPO should immediately address it otherwise it may appear they’re benefiting from it. It’s their conflict of interest as well. VA is failing patients and health systems in the USA. Patients in the quality of care failures represented as compared to other high income countries. Cost, as compared to what the USA spends on supplies when compared against global spend. I concede that VA isn’t responsible for all of it. But, they are the front line on product selection. Product selection is where quality & cost begins! What’s wrong with demanding leadership on cost & quality from these professionals? What’s wrong with expecting action that’s openly communicated & that demonstrates a shift towards a results based operation with a baseline stated. What’s wrong with demanding the conflict of interest be immediately addressed by the GPO and AHVAP? What I see from them is congratulations on new certifications. I believe that’s membership revenue…is that what they care about? Who cares who’s certified if our patients are suffering & our health systems are shutting down services/rural hospitals closing because they have cost problems so severe they can’t afford to hire, or keep their doors open! Supplies; the second highest cost in a health system next to employees, and fast approaching number one.

*Market Update* NHS England has issued a reminder to health service leaders about the mandatory submission of national returns during the implementation of Electronic Patient Record (EPR) systems. The letter, sent to executives and digital leads at NHS Trusts, emphasises the importance of maintaining an understanding of patient needs and the importance of effective reporting to support financial, operational, and clinical quality oversight during EPR implementation. It cites instances where providers halted critical reporting for extended periods after EPR go-live and stresses the requirement to submit mandated returns as part of their provider license. The letter also outlines additional governance measures introduced for EPR go-lives to ensure business continuity and formal approval processes ahead of each deployment.
