At the recent 2024 PQA Annul Meeting, Stephen Jones, VP Advanced Analytics, joined Sheila Botts, Regional Administrator, Clinical Pharmacy Services at Kaiser Permanente, Colorado and Vanessa Campbell, Director, Clinical Pharmacy at UPMC Health Plan to talk about unfavorable social-risk factors (SRF) and how they negatively impact medication adherence and health outcomes. Working with our payer partners, RxAnte developed a supervised machine learning model to identify at-risk SRF cohorts. Tailored outreach was developed by our clients to identify and address barriers to medication adherence and other health behaviors. Medicare Advantage programs have grown rapidly, and a shift toward more medically complex beneficiaries has implications for health care utilization, costs, and quality in the MA program. Ensuring safe and effective medication use in this population has been identified as a priority, yet there is limited evidence to guide MA plans’ pharmacy care management efforts. The goal of this study was to investigate the impact of an integrated PCM program for MA beneficiaries taking multiple chronic medications, including a subset with high predicted cost savings from improved medication adherence, assessed using the Value of Future Adherence score. Using adjudicated administrative claims data from a regional Medicare Advantage and Part D plan, a retrospective matched cohort study was designed. PCM program enrollees underwent monthly medication reviews, reconciliation, and synchronization of refills, which were dispensed and delivered to the home in adherence packaging by a specially equipped pharmacy. Members were contacted monthly and received, as needed, education, referral to health care providers, consultation with prescribers on changes to medication regimens, and referral to social support or case management programs. Primary outcomes of interest were per enrollee per month costs, resource utilization, and medication adherence for three MAPD Star Ratings measures over 12 months for all members and for the subgroup with a high VFA score. Among all PCM program enrollees, there was a PEPM increase in average pharmacy spending compared with control members and an offsetting decrease in medical spending, resulting in PCM participants having lower total cost of care, fewer inpatient stays, and improvements in medication adherence. The Centers for Medicare and Medicaid have ahead of them the goal of getting 100% of Medicare (and the majority of Medicaid) beneficiaries to be enrolled in some type of accountable, or value-based, care arrangement by 2030. At the heart of these arrangements are patient outcomes which can only be measured if organizations have access to comprehensieve and efficient patient data. However, three barriers – takeholders speak different data languages; data isn’t mapped consistently; and data isn’t interpreted congruently between providers and payers – are keeping us from interoperability.
