With support from Rocky Mountain Health Plans, St. Mary's Regional Hospital (now Intermountain Health St. Mary's Regional Hospital), launched the Integration to Minimize Potentially Avoidable Costs (IMPAC) Program to help drive improved health outcomes and patient experiences for people with co-occurring physical and behavioral health conditions and to reduce avoidable ED utilization. In its first year, the program saw a 66% decrease in ED visits across the combined group of patients, and patients reported high satisfaction with the program and an increase in their health care needs being met. Addressing Social Determinants of Health: A Call to Action Have you ever faced the tough choice between buying food for your family or purchasing necessary medications? Or struggled to access healthcare services for your children due to lack of transportation? These dilemmas are everyday realities for many across the United States. As healthcare professionals, we are acutely aware of these challenges and how they impact cost of care, patient outcomes, and quality of life. Years ago, we simply called it “non compliance.” I still remember when we charged patients for missed appointments and often turned them away if they were 15 minutes late, regardless of the reason. Many people, healthcare providers, government agencies, and nonprofit organizations have been working hard to change the narrative by creating amazing programs like free transportation services, subsidized medication programs, food as medicine initiatives, and improved access to healthcare with virtual appointments and remote device monitoring strategies. However, the challenge remains. This is largely due to ineffectively connecting these services with the individuals who need them most, often due to a lack of awareness from both service providers and recipients. Implementing systematic screenings for social determinants of health allows us to proactively identify individuals and families in need and navigate them to the appropriate resources. By ensuring that basic needs are met, we empower individuals to reach their full potential, embodying a truly person-centered approach to healthcare. Deploying digital solutions, leveraging AI and combining that with human empathy and guided support we can make a difference on a much larger scale. Let’s create healthier communities together. Let's continue to work to address these fundamental issues and transform our healthcare system into one that recognizes and acts on the broader social factors affecting health. Today we released our 2020-2022 Impact Report, "On the Road to NACHC of the Future," which captures the work of NACHC and the Community Health Center Movement during a unique time. For Community Health Centers, this period was marked by unprecedented challenge and change. Health centers demonstrated the strengths of a nationwide network of community health providers by confronting the challenges presented by the COVID-19 pandemic head-on in rural and urban medically underserved communities. 🌟 Happy Social Prescribing Day! 🌟 Two years ago, I embarked on a journey with #socialprescribing that has since transformed my professional pursuits. As a founding member of the International Social Prescribing Pharmacy Association (ISPPA) and the Canadian Social Prescribing Student Collective (CSPSC), which is under the umbrella of the Canadian Institute for Social Prescribing, I dedicate much of my time to promoting this #holistichealthcare among #pharmacists and the #student community. At ISPPA, we're championing the integration of social prescribing into pharmacy practice, empowering pharmacists to offer patients pathways to non-medical community support. We are also working with stakeholders globally to visualize why #pharmacy is ideal for identifying social prescribing needs in communities across the globe. My work with CSPSC further extends this passion by focusing on publishing research highlighting the benefits and implementation of social prescribing among students and educating students through conferences and webinars. This dual role has allowed me to advocate for a broader recognition of social prescribing's role in addressing health holistically, especially within academic settings.
