It is true, a person’s genetic make up affects how they respond to certain medications particularly central nervous system (CNS) neuromodulators. In treating chronic pelvic pain patients and Endometriosis patients at PRM, we sometimes use GeneSight to analyze a patients genetic make up and response to medications we are considering prescribing and tailor the treatment to the individual. However, at Pelvic Rehabilitation Medicine, our mission is to use the PRM Protocol™, a safe, office-based procedure that is a functional and restorative approach to pelvic pain, and add as little CNS neuromodulator medication for the shortest amount of time possible to minimize medication overload. https://lnkd.in/e5Y8ansU

Pain mechanisms should always be considered in rheumatology research! Researchers from the Parker Institute are part of global initiatives exploring how different pain mechanisms (i.e., nociceptive, neuropathic, nociplastic) impact different health outcomes - modifying, confounding and potentially even bias the outcome measures applied across various aspects of Rheumatic Musculoskeletal Diseases. No clinical trial studying treatments for patients with painful CIAP has been yet executed, thus only extrapolation of the value of treatments from other neuropathic pain fields is possible. Clearly, the pathogenesis and pathophysiology of CIAP will be quite different from that of PDN. Many CIAP patients complain of intolerable side effects of current analgesic medication. In order to help those patients, we developed a treatment-regime consisting of the supplement palmitoylethanolamide and topical formulations of analgesics. This combination is rarely complicated by side effects. One of our most recent topical formulation we developed contains 10% phenytoin, a broad acting voltage-gated sodium-channel blocker. Analysis of biorepository data shows that patients with a diagnosis of #psoriasis (PsO) at an older age have a shorter interval between PsO and psoriatic #arthritis (PsA) diagnoses and are more likely to have concurrent diagnoses compared with patients with an onset of PsO at a younger age. Nociplastic pain (NcplP) does not equal central sensitization: NcplP is defined as “pain that arises from altered nociception despite no clear evidence of actual or threatened tissue damage that causes peripheral nociceptors activation or evidence of disease or lesion of the somatosensory system causing the pain”. Numerous studies have shown that nociplastic pain is present in many different diseases such as fibromyalgia, complex regional pain syndrome type 1, and irritable bowel syndrome. Known mechanisms can be divided into three categories: First category - supraspinal mechanisms: hyperresponsiveness to pain stimuli, hyperactivity, and connectivity between regions of the brain responsible for pain perception; decreased activity and connectivity of brain areas responsible for pain inhibition Second category - spinal mechanisms: regional clustering and convergence of signals from different pain loci, spinal cord reorganization, amplified spinal reflex transmission, decreased spinal inhibition, winding up and temporal summation, and immune system activation. Third category - peripheral mechanisms: proliferation of sodium channels and sympatho-afferent coupling. In clinical practice, pain signaling involves main compartments that can contribute to the development of pain in all three sites.
