Upon Theresa’s initial evaluation she displayed a high fear of falling and decreased tolerance for functional mobility. Theresa was frustrated that she could not get out of bed and felt she lost a lot of her independence, however, she was hopeful and motivated to learn. During the first 4 weeks of Theresa’s rehab she required a mechanical Hoyer lift to transfer in and out of bed. As she got stronger due to physical and occupational therapy, she was able to go from laying down to sitting at the edge of the bed with 2 people; however, sitting on the edge of the bed was extremely difficult. Theresa was seen by our speech language pathologist to improve her sequencing and processing. A lot of time was spent encouraging and educating her to make sure she felt safe and understood. Theresa’s supportive family worked closely with our rehab team to ensure she was optimizing all her therapy time. As Theresa grew more comfortable with her therapists, she was able to transfer into her wheelchair with 2 people with use of a rolling walker. She started to get into a routine of getting out of bed daily with just one person and was inspired to improve her balance and strength; eventually learning how to walk again. Here at DISH, our speech therapy team works closely with our Occupational therapist to maximize our clients' arousal levels and get them ready to learn. You can do the same with your kids at home! If your child has low arousal, try novel activities. Sing a new song or play with a toy in a different way than you typically would, such as hiding Mr. Potato Head parts around the room and going on a search for them. Play games with unpredictable movement. Vary your speed during jumping, bouncing, spinning, or swinging. Provide strong input such as spinning, upside-down movement, or tickles. If your child has high arousal, engage them in familiar games with slow, smooth, rhythmic movement. Speak with a soft voice. Decrease the stimulation in the room (for example, dim the lights and cover toys that are not being used) or choose a less stimulating environment (a quiet room with no other people and few toys). Provide calming input such as rocking back and forth, pillow squishes, or arm squeezes. Occupational therapists working from a sensory integration frame of reference focus upon how a child is able to take in information from the environment, organize that information and then respond and adapt to their surroundings. We can think about a child’s development as an inverted triangle with sensory systems on the bottom, precariously balancing all the rest of our higher-level functions, including speech and language skills. Early sensory experiences help to shape our motor skills and how we respond to our environment. The input we receive early in life is extremely important. As infants, the connection between the hand and the mouth is one of the first things we learn. We then develop oral motor reflexes like rooting and sucking and eventually begin to regulate ourselves. Moving our arm and hand provides feedback from our muscles and joints and begins to give us our sense of “self.” When an infant cries and their needs are met as a result, this is our earliest back-and-forth interaction and the beginnings of communication. The vestibular system receives information from within our inner ear, which travels in two directions: up to the cochlea and the visual system and down to our proprioceptive and tactile systems. This input helps maintain balance, affects our ability to sit still, and controls eye movement. The vestibular system coordinates and controls our movements, telling our body when we are moving, how fast or slow we are moving, and when we stop and start moving. Similarly, speech production involves motor control and motor planning of fine motor muscle movements. The vestibular system works closely with the auditory system; both receive information from the receptors in the ear, and are separated anatomically by only a bony labyrinth. When sound passes through our ear, it creates vibration. This movement is what our body understands, not the sound itself. If a child’s body struggles to understand this movement, they may also struggle to process sounds.
