I was lucky to be asked to help explain perioperative pain management in total joint arthroplasty cases by Joe Paone at AORN. Great commentary by Charles Hannon, MD, MBA on how we can take better care of these patients. Kudos to Justin T. Deen, MD MBA who led a lot of the research that is discussed in this article. Low back pain is one of the most common disorders on the world. To overcome low back pain, we should first understand the mechanisms behind it. Paraspinal muscles, specifically multifidus, has grabbed attention as a cause for low back pain in recent decade. Fatty infiltration of multifidus has been related with low back pain. In our recent paper published in Pain Practice (official journal of World Institute of Pain), we depicted that fatty infiltration of multifidus is present in even asymptomatic subjects. What was different between symptomatic and asymptomatic subjects, then? It was fatty infiltration of erector spinae of upper low back.
