👇Totally agree. With the large gap in primary care we’re facing, we need more flexibility and support of physicians that can fill some primary care roles. A big part of my day in the emergency department is doing primary care. Explaining results and arranging follow up. When I started emergency medicine residency, I didn’t want to do primary care, and often would say, “follow up with primary care,” without engaging to see if that was realistic for my patient. Now I seek to understand what the reality is for my patient around primary care and fill the gap in the best way I can. I now find these moments really meaningful. Many emergency doctors reach mid career and want more regular hours. Not everyone wants to switch into urgent care. I’ve thought for years that adding an emergency doctor to a primary care office could be a win, win. We could see add on patients, do procedures, and prevent some ED trips. Many doctors feel trapped in their specialty and leave medicine. We need to recognize the value of their education and experience. Let’s find innovative ways to create more win-wins for patients and doctors. #healthcareinnovation #primarycare #emergencymedicine

A nice overview of digital #primarycare and how growing #telemedicine has resulted in needing to think through accountability. Trust has been eroding in the healthcare system. One solution offered by this author is having a virtual primary doctor and ensuring there is more accountability instead of piecemeal care. At #atanexus, there were a number of discussions on the growth of telespecialty care especially with #VBC models and improved reimbursement. Like everything, telemedicine improves access to specialists. The author's concern is that, as a result, patients are often seen by their specialist most often and may not have a PCP to keep all their health data centralized. The role is crucial as primary care physicians help guide patients as they gather information from specialists, hospitalizations etc. It is not easy to sort through all the information and make a decision. In this way, virtual PCPs can be a vital use case. The question is if we have enough PCPs and it can be reimbursed and staffed. Since in this day and age one cannot fail to mention #AI without being immediately booted from the planet, primary care is getting in on it also. Most are similar to other specialties such as treatment plans and tailored intervention. But also for identifying high-risk patients and intervene proactively to prevent complications or emergencies. I am not a primary care doc but have many friends working on expanding digital primary care. I have seen most of digital health focusing on specialty care, #emergency care and image based solutions.
