A new technique for accessing the mitral valve via the left anterior minithoracotomy has been developed for patients requiring both mitral valve surgery and coronary artery bypass grafting. Immediately sequential bilateral cataract surgery (ISBCS) has numerous benefits. The most feared complication is bilateral simultaneous postoperative endophthalmitis (BSPOE), which is extremely rare. In JAMA Ophthalmology, we present the first ever recorded outbreak of BSPOE after ISBCS. Although this tragic event had severe consequences in two eyes, all patients recovered good visual function in at least one eye. A clear breach of sterility or patient selection recommendations has characterized almost all known cases of BSPOE. Fortunately, the risk of getting BSPOE after ISBCS is still lower than dying in a car crash after driving 100 meters in the USA. In conclusion, ISBCS is safe, but strict adherence to guidelines should always be followed.
