I am constantly amazed how the collection of people who make up Housecall Providers have such an outsized impact on our world. When people tell you an organization is "mission driven", it's worth discovering what they mean by that. And, if what they mean is that they truly do belong to a mission driven organization, and you support that mission, then support that organization. As someone who coordinates with donors and engaged community based organizations, I can tell you with solid assurance that community support energizes truly mission driven work. The value of volunteering, donating, and/or working with visionary organizations goes far beyond the individual connections of the act. It's not that any one organization is going to save the world. It's that supporting organizations that are actively making an impact and are authentically mission driven is a habit that collectively can change the world for the better.
We had a wonderful time at The Coalition to Transform Advanced Care (C-TAC) - Center to Advance Palliative Care (CAPC) Leadership Summit in Washington, D.C. this week. It was inspiring to connect with so many national changemakers focused on advancing person-centered care and improving quality of life for people living with serious illness and their caregivers. New rules from the Centers for Medicare & Medicaid Services will further accelerate community engagement efforts across the nation. New findings on effects of IMPaCT Care CHWs from Patient-Centered Outcomes Research Institute (PCORI) show significant impacts on hospitalizations and primary care engagement. We’ve known for a long time that structured CHW interventions improve equity and drive value. Now, we know that they can do it at scale. Kudos to Ballad Health for a statistically significant 29% lower rate of admissions, demonstrating the power of CHWs to move the needle in rural areas. CHW programs aren’t just a community benefit; they’re an important population health strategy.
At IMPaCT, one of our values is to “learn, constantly.” That’s why we conducted three RCTs of our structured Community Health Worker intervention, and it’s why we participated in a PCORI-funded implementation evaluation of our structured CHW program at five sites across the country. Drumroll please! The findings - https://lnkd.in/gWCQ3cGi - demonstrate that we can achieve RCT-level results at scale. Three sites demonstrated high fidelity and achieved significant reductions in utilization, and all are continuing and expanding their programs. At Ballad Health, an integrated community health improvement organization serving 29 counties of the Appalachian Highlands, patients who received IMPaCT had 29% fewer hospital stays compared with similar patients who didn’t receive IMPaCT. These impacts illustrate the transformative power of community health workers in a variety of settings. We are thrilled to work with more than 50 partners in 20 states to deliver results like these at scale. Medicare coverage of CHW services represents important progress in ensuring that CHW services are accessible and sustainable, but there is more work to be done. We appreciate the work Senator Casey has done to advance access to CHW services through the CHW Access Act, and we hope to see more state Medicaid agencies taking action to ensure that evidence-based CHW services are available to the communities that need them most.

A patient is referred to hospice care when a doctor’s prognosis is six months or less and you might think that means that after 6 months care ends. The reality is that hospice care can continue as long as the need for hospice care remains. World Hospice and Palliative Care Day 2023 (WHPCD) is a unified day of action to advocate for the support of people with serious health related suffering, including those at the end-of-life, caregivers and their families, and to support the presence of hospice and palliative care programs around the world. Over 56.8 million people are estimated to require palliative care every year including 31.1 million prior to and 25.7 million near the end of life. 67% are adults over 50 years old and at least 7% are children. The burden of severe illness and health-related suffering, and the need for palliative care, are immense. Yet palliative care is still not accessible to most people in need, especially in low and middle-income communities. This issue is especially important to Tenacity after completing our work with Rebecca Swain-Eng, MS, CAE of SEA Healthcare and the Maryland Department of Healthcare - Maryland Health Care Commission where we implemented and analyzed a palliative care study surveying over 300+ palliative care programs to make recommendations for improved access to quality services for all. #WHPCD2023 #PalliativeCareDay #HealthCareEquity #PalliativeCareAwareness #PalliativeCareResearch #Partnership
