Are categories a hindrance to good root cause analysis? Some people seem to think that the only way to arrive at a root cause (or root causes) for an accident is to do original thinking for each investigation and not rely on pre-defined categories. Many people with this view of problem-solving complain that any system with “categories” limits investigators to a pre-defined view of the world and, therefore, causes “less good” root cause analysis than their “unconstrained” view of the world. Got an upcoming investigation report that is due? Make sure you have included these 5 things before submitting it for approval: 1. Background or Process Description - Make sure you described the process you investigated. 2. A chronology of events - This is to show when and how things happened. 3. Justification for how you ruled in and ruled out all the potential causes you identified. 4. A conclusion of your root cause. 5. A CAPA Plan - There shall be a corrective action for each root cause.
