THEORETICAL QUESTION: There is a medical treatment that you came across and are considering adopting in your practice so you start researching the published data and you come across conflicting reports: 1. On the one hand….you find published data from multiple industry sponsored clinical trials that are well conducted and executed, positive long term data and good overall safety - also you know many of the authors on these publications 2. On the other hand….you find manuscripts using meta-analysis and Cochran reviews which exclude any industry sponsored data and report that this particular therapy doesn’t work, the authors are people with no COI’s relevant to this particular therapy and you have never heard of any of them. Most of your colleagues have already started to adopt this new therapy and many seem to like it… What do you do? 🔍 From bench to bedside: The new issue of the magazine "Aus #WissenWird Gesundheit" sheds light on translational aspects of medical research. https://lnkd.in/ecXPcvgp What does it take to make findings from the Petri dish usable for patients?Ingrid Fleming emphasizes the importance of networking in basic and clinical research. At the Cardio-Pulmonary Institute we value and use our network in our daily work. In the magazine you can also read a successful example of our research on the aging heart: A team led by Stefanie Dimmeler and Julian Wagner was able to show that the growth of nerve cells in the heart muscle is inhibited in old hearts by the release of a messenger substance. As a result, the heart muscle cells can't be informed that they should work faster in order to pump more blood through the body when needed. The heart is therefore less able to respond to physical stress. There are indications that the messenger substance responsible for this can be suppressed with medication. 𝐏𝐫𝐞𝐜𝐢𝐬𝐢𝐨𝐧 𝐦𝐞𝐝𝐢𝐜𝐢𝐧𝐞: 𝐓𝐡𝐞 𝐂𝐨𝐧𝐯𝐞𝐫𝐠𝐞𝐧𝐜𝐞 𝐨𝐟 𝐇𝐞𝐚𝐥𝐭𝐡 𝐚𝐧𝐝 𝐌𝐞𝐝𝐢𝐜𝐢𝐧𝐞 In this week's Radical Friday piece, we explore the topic of precision medicine and its growth beyond cancer therapy, reaching into the area of other non-communicable diseases. This approach isn't just for one; it's about creating precise care pathways tailored to each individual's needs, taking into account the diversity among us, such as different ethnicities responding to treatments for conditions like hypertension. Precision health is stepping up too, moving away from broad-stroke strategies to more nuanced care that aligns with people's aspirations and their commitment to shifting towards healthier choices. However, there seems to be a persistent view that precision health and precision medicine require distinct approaches. Medicine traditionally focuses on correction, while health emphasizes prevention. But is this separation necessary? Precision medicine is deeply rooted in the Electronic Medical Record (EMR), the crucial document clinicians use to record patient care, holding significant medicolegal weight. In contrast, precision health is often a tapestry of personal and public health records, enriched by data from wearable devices—though authored by citizens, not clinicians, and occasionally less accurate than traditional medical devices. Yet, we often consider wellness data as merely complementary to medical data. Shouldn't we reconsider this stance, given that the management of chronic diseases is a lifelong journey? A valuable lesson from experience is that academic debates about the definitions of mild versus moderate disease and what constitutes "normal" can overshadow more practical considerations. It's time to shift from binary to a spectrum of health, recognizing that most of us have some form of health issue that we live with. As we advocate for lifelong disease management, we should also advocate for a seamless integration of health and medicine. 🎉 We’re delighted that our latest study on the impact of gender, ethnicity, and social deprivation on access to surgical or transcatheter aortic valve replacement in #aorticstenosis has been published by OpenHeart, an open access journal by BMJ and The British Cardiovascular Society, which publishes original research in #cardiovascular medicine. Utilizing anonymized real-world linked clinical datasets, our analysis revealed that people with aortic stenosis of female gender, Black or south Asian ethnicity, and higher deprivation had a lower chance of receiving an aortic valve replacement. Furthermore, our research highlighted that "timeliness" of aortic valve replacement differed by ethnicity and deprivation.
