This is another area most important for the 40% of Americans most behind in 2621 counties lowest in health care workforce with 45% of mental health needs and 45% of other complexity yet the financial design allows only 25% of each workforce in mental health, primary care, and women's health supported poorly by less than 20% of spending in each specialty (also only 10 - 13% of geriatric and child psych workforce, and 15% of geriatrics and psychiatry workforce) 1. Requires broadest scope highly experienced highly retained primary care, which is failed most as our nation moves to the least experienced most narrow primary care workforce in its history with more and more rapid turnover and departures from needed locations and from primary care 2. Forces more complex duties on local primary care from all team members and their spouses and families across practice, community leadership, and community health leadership (These are activities and duties which CMS has been killing off for 40 years of DRGs and lower payments closing and compromising hundreds of hospitals and thousands of practices in these counties in addition to workforce, finances, jobs, and social determinants) 3. despite lesser payments by 15% as documented in Medicare 2011 data (which was supposed to embarrass physicians abusing Medicare, but exposed Medicare to its failure to audit and be accountable since it pays less and less as workforce levels get lower - worsening deficits of workforce and access by design 4. despite the worst quality worst paying health plans - the ones that fail most where there are concentrations of elderly, poor, disabled, and worst employers as in 30 states and in these 2621 counties lowest in workforce - BY Financial design (no training design can overcome this financial design - zero, zip, nada, not new types of graduates or massive expansions in 3 sources or pipelines) 5. despite fewer delivery team members and lesser delivery team members because of the finances, with lower productivity, higher turnover, burnout and worse. And the finances are made worse by increasing costs of delivery in old sources with new sources added. And failure to compensate for usual disruptions, smaller size practices with additional costs, or concentrations of the worst plans. This is what Congress, CMS, states, and private payers have always failed to address - and have made worse with 40 years of innovations, regulations, digitalizations, micromanagements, and cost cutting as seen from DRG to value based designs. Revenue losses directly and also lost procedures, OB, hospital, ER, and assistant surgery kill basic health access. RBRVS kills basic health access. Failure to audit kills basics. Penalties kill hospitals and practices serving the most complex despite the least support facing the highest complexities. The only consistent thing about value basis, is the lack of valuing primary care and most Americans most behind
