The AHEAD Model is the latest total cost-of-care model from CMS, with goals of slowing growth in health care costs, improving population health, and advancing health equity. Primary Care AHEAD is a key component of the model that aims to help states increase investments in primary care and support advanced primary care initiatives through capacity building.
