The Symphony Begins: Atria Contraction: The cardiac cycle commences with a harmonious symphony as the atria, the heart's upper chambers, contract. This contraction exerts pressure, propelling blood into the ventricles, the lower chambers. The atria's synchronized effort ensures efficient blood transfer and primes the ventricles for their crucial role. 2. Powerhouse Performance: Ventricular Contraction: As the atria relax, the spotlight shifts to the ventricles, the heart's true powerhouses. They take center stage, contracting forcefully to pump blood out of the heart and into the arteries. 🚨 𝐂𝐚𝐧𝐜𝐞𝐫 𝐕𝐚𝐜𝐜𝐢𝐧𝐞 𝐋𝐚𝐧𝐝𝐬𝐜𝐚𝐩𝐞 💉 𝐀𝐜𝐜𝐞𝐬𝐬 𝐏𝐃𝐅: https://lnkd.in/dRvMfgTJ Cancer vaccines work by “educating” a patient’s immune system against a certain target that is known to be present in the tumour, called an “antigen” The way this is achieved is by exposing “antigen-presenting cells”, like dendritic cells (DCs), to antigens. The APCs will then go show (or present) the antigen to cytotoxic T cells, and say, “hey! Go kill THIS thing”. 𝐓𝐡𝐞𝐫𝐞 𝐚𝐫𝐞 𝐭𝐰𝐨 𝐦𝐚𝐢𝐧 𝐭𝐲𝐩𝐞𝐬 𝐨𝐟 𝐜𝐚𝐧𝐜𝐞𝐫 𝐯𝐚𝐜𝐜𝐢𝐧𝐞𝐬: ➡️ Nucleic acid-based (mRNA/DNA) – these rely on the infusion of mRNA/DNA-encapsulating nanoparticles that will bring tumour antigen-encoding nucleic acids to antigen presenting cells in vivo. These APCs will then express the antigens and present them to T cells in lymph nodes. 𝐏𝐫𝐨𝐬: quicker, cheaper, easier to manufacture and infuse. 𝐂𝐨𝐧𝐬: number of antigens is limited. It’s also impossible to educate against unknown, patient/tumour-specific neoantigens. ➡️ DC-based – these are (normally) autologous platforms that extract a patient’s own DCs, expose them to lysates from their own tumour – priming them against tumour antigens, including patient-specific neoantigens – and then reinfuse into the patient. These tumour-educated DCs will then go and present tumour-specific antigens to T cells in lymph nodes. 𝐏𝐫𝐨𝐬: “unlimited” antigen repertoire – this is significant as it educates host immune system against a vast array of antigens including unknown, tumour-specific neoantigens (incredibly personalised) 𝐂𝐨𝐧𝐬: complicated manufacturing, expensive, longer, less consistent cell product (patient health can affect therapeutic efficacy for example). 𝐒𝐨, 𝐢𝐧 𝐬𝐮𝐦𝐦𝐚𝐫𝐲, nucleic acid-based vaccines have a “simpler” design and are easily manufactured but they are significantly less robust than DC-based vaccines in terms of their capacity to present diverse antigens. In the first half of 2024, we have 6️⃣ FDA approvals of new indications for cell and gene therapies, which already makes this year officially the most fruitful ever from this angle. 1️⃣ Breyanzi (from Bristol Myers Squibb) provides deep and durable responses for patients with relapsed or refractory CLL or SLL who have historically hadn't standard care. In TRANSCEND CLL 004, pivotal multicenter trial to evaluate a #CART cell therapy in patients with relapsed or refractory CLL or SLL, 20% (95% CI: 11.1-31.8) of patients treated with Breyanzi achieved a complete response (CR) with an established safety profile. 2️⃣ Abecma (from Bristol Myers Squibb & 2seventy bio) tripled progression-free survival (PFS) compared to standard regimens in the Phase 3 KarMMa-3 trial, with a 51% reduction in risk of disease progression or death and a well-established safety profile - median PFS of 13.3 months (95% CI: 11.8-16.1) vs. 4.4 months (95% CI: 3.4-5.9). 3️⃣ Carvykti (from Johnson & Johnson) approval is based on results from the Phase 3 CARTITUDE-4 study. The median PFS wasn't reached in the Carvykti group and was 11.8 months in the standard-care group. PFS at 12 months was 75.9% (95% CI, 69.4 to 81.1) in the Carvykti group vs. 48.6% (95% CI, 41.5 to 55.3). It's first and only approved #BCMA-targeted treatment for patients with relapsed or refractory multiple myeloma who have received at least one prior line of therapy. 4️⃣ The approval of Lenmeldy (from Orchard Therapeutics - U.S.) is based on data from 37 pediatric patients with early-onset MLD, enrolled in 2 clinical studies or treated under European expanded access frameworks, who received a one-time administration of the gene therapy and compared with natural history data. With more than 12 years of follow-up in the earliest treated patients (median 6.76 years), #treatment with Lenmeldy significantly extended overall survival and resulted in the preservation of motor function and cognitive skills in most late infantile MLD patients. 5️⃣ A one-time dose of Beqvez (from Pfizer, developed by Spark Therapeutics, Inc.) has reduced bleeds post-treatment compared to standard of care after up to 3 years of follow-up. The approval is based on the results from the pivotal BENEGENE-2 study to evaluate the efficacy and safety of Beqvez in adult male 45 participants with #hemophilia B. Bleeds were eliminated in 60% vs. 29%. A median ABR of zero (range of 0 to 19) was observed during the efficacy evaluation period compared to the prophylaxis arm in which a median ABR of 1.3 (range of 0 to 53.9) was observed. 6️⃣ By a throw of the belt, Kresladi (by Rocket Pharmaceuticals), whose approval was delayed due to an extension of the regulatory agency's review of the dossier until the end of June, got into the list. Taking into account, approved at the end of last year, Casgeva's gene therapy (Exagamglogene autotemcel) from Vertex Pharmaceuticals, we can talk about a growing trend for modern #CGT precision medicine.

Oncology is the largest market in BioPharma, worth >$200B per year. While the top medicines in the field have remained relatively consistent for some time, a big re-ordering will take place in the next few years: Powerhouses like Keytruda, Opdivo, and Revlimid will see key patents expire in 2-4 years. Keytruda and Opdivo will have more protection than Revlimid given they are harder to copy biologic medicines, and will thus benefit from more gradual sales declines. While Keytruda does have an LOE this decade, it continues to perform at an astounding level - gaining $1.1B more in sales this past quarter than the comparable one last year, almost defying the law of large numbers. Meanwhile, newer generation medicines like Enhertu are showing strong sales growth and powering up the ranks. The strength of AstraZeneca's oncology pipeline comes through - not only does it make several of the fastest growing drugs, many of these products have relatively long patent lives remaining, with Tagrisso, Imfinzi, and Calquence enjoying patent protection past 2030. Another medicine worth highlighting is Lilly's Verzenio, which crossed the $1B sales mark in the first quarter. Much of the attention on Lilly often focuses on its GLP-1s, but the company maintains a strong oncology pipeline as well. And while there are many promising medicines, it is worth remembering the tremendous time and expense required to get an approval in oncology: Verzenio is a case in point - despite being a success in breast cancer, it recently flopped in a phase 3 trial for prostate cancer, costing Lilly $75M, which underscores the road to the breakthrough drugs we all crave is paved with setbacks and requires tremendous, consistent investment.
