Most companies claim to design products or services that are simple to use. But when you ask customers to actually use these products and services, they often find them far from simple. Why is there a disconnect between what organizations think of as “simple” and what users actually experience?

Oh, this is excellent - this is exactly why I do things the way I do, and it's 100% contextual.
If I'm delivering usability test results with 5-6 users in every sprint for the same team, you'll get a couple of graphs and maybe a paragraph of explanatory text with a few bullet recommendations.
If it's 90-minute depth needfinding interviews with lots of users then you are absolutely getting a hundred-page brick (with a summary at the top) - especially if you're in a non-integrated model and the team is, shall we say, very hands off. If they're not responding to messages and follow-ups, you probably won't be able to get them into a workshop, so you have to throw the brick over the wall and run.
My much-preferred way of doing things is to run a workshop with the whole product team (engineers, marketers, design, everyone) and get them to fill out problem charts: what the customers need, what they're doing now, and what we need to offer them to do better.
That right-hand column then becomes the requirements list for the next phase of development, and since they came up with it themselves, they're more likely to act on it.
Even so, I usually also provide a long-form report and make shorter summaries:
★ You can allow your work to be scrutinised - if anyone questions the findings, they can see for themselves how you got there
★ You can repackage and repurpose your work in different forms for different audiences - much easier if nothing was lost in the first place
★ You can cut a traditional report into a lean report, and then again into an infographic or visual summary.
Everyone has different preferences, so the more material you start off with, the easier it is to cut it down to a size that fits.

Some claim that reports are an essential part of the UX research process, and others claim they’re a needless waste of time. What accounts for this difference?
In her 2023 UXPA talk, “The report is dead, long live the report”, Kathi Kaiser breaks down the factors that influence the need for, and effectiveness of, investing time in research reports. These factors include the team model, the level of involvement stakeholders have early in the research, and the formatting of the report.
In short, how we communicate research findings should be based on our team context.
