I enjoyed the perspective that Swee Kheng Khor took on this: I agree in parts, although I have a couple of extensions to offer. Firstly, social health insurance seems to me intuitively a good deal for everyone. Health insurance providers benefit from expanding the pool - at least up to a point, and we should be mindful of the examples set in other domains. Courier companies are often more profitable than postal services because the latter has to serve EVERYONE. Once you socialise a service you have to recognise you have dual targets of private profit AND public good. Secondly, adding savings and investments to insurance could make these more appealing, but I am cautious. I see a lot of investment-linked products with low returns and high fees; people want and need help overcoming their present bias to save for the future, but in some cases the price seems awfully high. It would be great to see insurance adding on more passive products that allow both insurer and insured to share the returns. Finally, our prior research suggests one of the challenges with Malaysians being under-insured is that the current market has too much assymetric information and choice overload - we need to do more to help Malaysians make good choices. [ICT Systems & Solutions for Long-Term Care Providers in Japan: Key Research Findings 2023] Yano Research Institute (the President, Takashi Mizukoshi) carried out a survey on the domestic market of ICT solutions for long-term care providers and ICT solutions for the elderly care (watch service for the elderly at home, watch service for long-term care facilities), and found out the market size transitions, trend of market players, and future perspectives. This press release discloses the market trends of ICT solutions for long-term care facilities. Summary of Research Findings: Domestic demand for ICT solutions at long-term care facilities is mostly renewals (updating of an outdated system). When opening a new facility or management office, or when their management corporations go through mergers, etc., long-term care providers renew their contract or sign a new contract. Our estimate on the size of the ICT Systems & Solutions for long-term care service providers (based on the sales of businesses) was 25,213 million yen in FY2022, 102.8% of the previous year. While the benefit of governmental financial relief during the COVID-19 crisis diminished, deterioration of business environment led by care worker shortage and surge in prices of utility, fuel, food, and other products, caused long-term care service providers to postpone system deployment of any kind. Although the reclassification of new coronavirus to Category 5 infectious disease in FY2023 eased behavioral restrictions, ongoing price surge and labor shortage continue to strain long-term care businesses. In addition, given the revision of long-term care service fees upcoming in FY2024, long-term care service providers are likely to defer system deployment in FY2023. In 2023-2024, many insurers in Asia have launched significant new health transformation or commercial strategies, like Prudential, AIA and Manulife. 2. Insurers realize that Asia is under-insured with a $1.8T protection gap, and medical inflation outpaces general inflation by >40%. Insurers also see a commercial opportunity alongside their public duty. 3. Yet, insurers may still find it hard to adapt to emerging healthtech, innovation and analytics, on top of solving traditional principal-agent, adverse selection, and FWA issues. What to do? 1. Support national SHI trends. Introduce investment-linked products. 2. Use analytics and build political will to reduce medical inflation 3. Use innovation sandboxes and hackathons to successfully integrate new tech. Japan, Switzerland, Germany and other capitalistic democracies ensure for all their citizens access to quality affordable private insured and private manufactured FFS (fee for service) medical care with more private hospitals per capita and better outcomes and costs than in America.  They achieve this NOT by limiting or decapitating physicians reimbursement (Doctors with their staff earn only 7% of HC expenditures in America)  but by neutralizing the take of insurers, making insurers utilities instead of luxuries.
