Recently encountered a challenging case of airway management in a patient with traumatic brain injury (TBI) presenting sudden low Glasgow Coma Scale (GCS) and abnormal breathing patterns. Given the Lemon scoring or HEAVEN criteria pointing towards difficult intubation, ensuring successful airway management is paramount to minimize complications and prevent further deterioration, particularly concerning increased intracranial pressure (ICP) in TBI patients. In this scenario, adopting the RSII sequence became imperative, especially given the complexities of positioning within the ICU compared to the operating theater (OT). The plan involved utilizing video laryngoscopy as the primary method for securing the airway, with consideration of the Fremantle score for view, which unfortunately indicated none, and Cormack-Lehane Grade 3b with a POGO (percentage of glottic opening) of 0%. To enhance ease of intubation, modifications were made, and the McGrath series device was chosen. The procedure was successfully completed with the assistance of a bougie and the path of the Ryle tube serving as a landmark. This experience underscored the importance of meticulous attention to the airway and the aim of minimizing airway trauma.The ASA guidelines suggest using video laryngoscopy as the primary option for the first attempt in difficult intubation cases. #AnanthpuriHospital #NeurocriticalCare #ICUCriticalCare #AirwayManagement #medicaleducation

Is your hospital and post acute rehab setting managing shoulder subluxations correctly? Sure taping, slings, and proper positioning with pillows are important to keep the shoulder joint in an optimal position, but those will not FIX the problem! Best practice guidelines strongly supports the use of daily neuromuscular electrical stimulation to the shoulder muscles to reduce and/or prevent shoulder subluxation following an acute stroke or brain injury. Okay okay…I know what you are going to say… that’s good and all but it would be nearly impossible to carry out those guidelines in an acute hospital setting, right??? Nope. It’s not! On a Facebook page I follow, someone spoke of their experience receiving an ultrasound. They questioned the personal nature of some questions and wondered about the necessity. The person identified they tend to lean on the shy side. I am sharing my response here as it may be of value to some.... ------ For many when seeking and receiving care, our dignity can be the first casualty. Then, if you have a history of trauma/abuse, the experience can feel even more invasive and intrusive. While I don't know why you were asked, assuming necessary and helpful to the examination, I do hope it was carried out with some degree of empathy and concern for the experience. To the degree to which one feels comfortable, they can ask about the process or procedure from the person conducting it, prior to initiating. Again, to the degree comfortable, one can advise of any personal concerns before commencing. Hopefully, our dignity can remain intact, not compromised. I appreciated your asking the question.
